Position Summary

Under the direction of the Public Health Supervisor, the Community Health Worker serves as a liaison between the community and health care, government, and social service systems. The role focuses on building trust, empowering priority populations, and fostering collaboration with organizations and individuals. The Community Health Worker implements care plans and adheres to department policies, rules, and procedures.

  • The 2025 salary range is $23.43 to $35.14 per hour depending on experience.

Essential Functions/Responsibilities

Position Duties

  • Provide individual client visits and connect clients to resources that support positive health outcomes.
  • Assist individuals and families in managing chronic illnesses (e.g., diabetes, asthma, vascular disease).
  • Partner with public health nurses and case managers to address identified needs in client care plans.
  • Monitor client progress and provide follow-up support, including home and car safety checks, child growth tracking, and positive parenting education.
  • Complete billable and non-billable activities with accurate documentation for reimbursement.
  • Maintain compliance with data privacy laws, HIPAA, and Minnesota Data Practices Act.
  • Build and maintain professional relationships with community partners.
  • Act as a cultural bridge, offering insights on cultural responsiveness and implications in program activities.
  • Collaborate with community partners to conduct community health screenings and outreach activities.
  • Attend relevant training programs to enhance skills and knowledge.
  • Participate in Public Health initiatives, including quality improvement, performance management, and disease prevention efforts.
  • Prepare and submit reports, maintain client records in electronic health systems, and track program activities.
  • Attend staff and agency meetings as required.
  • Other duties as assigned to support the agency’s mission.

  • Knowledge of cultural, ethnic, and linguistic diversity and its impact on health.
  • Knowledge of policies, procedures, and privacy laws such as HIPAA.

Minimum Qualifications

Education and Experience

  • High school diploma or equivalent, at least three years of translating Spanish/English, and two years of healthcare or community services-related work; AND
  • Minnesota Community Health Worker (CHW) Certification from an accredited program (required within one year of hire).
  • Or an equivalent combination of education and experience sufficient to perform the essential functions of the job.

REQUIRED CERTIFICATIONS/LICENSES

  • Valid Minnesota driver’s license with proof of insurance

Working Conditions

  • Work is done in an office setting, community clinics, schools, client homes, and other community sites including, but not limited to, workplaces.
  • Frequent travel may involve exposure to inclement weather conditions, unsanitary conditions in homes.

Physical Requirements Of This Position

  • Desk work involving use of a computer, telephone, and other devices.
  • Communicate effectively with county staff, community partners and families needing services,.
  • Ability to read screens and documents.
  • Ability to drive in various light and weather conditions.
  • Ability to type and use a computer or mobile device.
  • Office work may include reaching for documents, grasping documents, standing, walking and repetitive motions.
  • Lift and carry files and material weighing up to 20 pounds.
