About the Position:
We are seeking a skilled and compassionate Community Support Coordinator to join our team. As a Community Support Coordinator, you will be responsible for providing support and assistance to individuals with intellectual and developmental disabilities.
Key Responsibilities:
- Obtain social information for the development of treatment plans and enrollment into Behavioral Health & Intellectual/Developmental Services under supervision.
- Assist consumers in filing applications for benefits and provide technical assistance to service providers.
- Participate in the development of Individual Service Plans detailing goals in areas such as residential day programming, socialization, medical, fiscal, and adaptive behavior.
- Maintain files, records, and other documentation necessary for continuity of consumer care and act as an advocate for consumers.
- Coordinate and direct meetings to assess consumers' progress in charted areas.
- Access supportive services for IDS clients residing in the community, including budgeting, shopping, leisure time activities, social skills, personal hygiene, and individual counseling.
- Attend resident reviews at State Centers, Residential Programs, Vocational Rehabilitation Facilities, and other service providers to ensure adequate levels of service.
- Provide crisis intervention and emergency services as coordinated by supervisor.
- Responsible for all other position-related duties not specified above.
Requirements:
- Must meet required minimum experience and training for this job: have successfully examined and have a current score on file for this job title with the State Civil Service Commission.
- A bachelor's degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science; or two years' experience as a County Social Service Aide 3 and two years of college level course work.
- Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service and one year of experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions.
