The County of San Mateo Family Health Services Division is seeking an experienced and qualified Senior Community Worker - Limited Term to serve in both the California Children's Services (CCS) program and Child Welfare Public Health Nurse (PHN) programs. Primary work location will be at 801 Gateway Blvd, South San Francisco, with some travel to other offices and locations in the community and as needed for program and client support.

California Children’s Services (CCS) is a State and County program for children and youth with special health care needs, from birth to age 21. As a medical case management program, CCS coordinates, authorizes, and in certain situations, pays for medical services and equipment for specific eligible health conditions, and provides diagnostic and treatment services for eligible children and youth. CCS also has a medical therapy program in San Mateo and San Bruno where occupational and physical therapy services are provided. The CCS program is subcontracted by the Health Plan of San Mateo (HPSM) to administer the care coordination and utilization management for HPSM Whole Child Model (WCM) members. The Children and Family Services PHN program supports the health needs of children and families by helping to ensure families and the children are able to access the services for their children’s individual health, growth and developmental needs. The team provides health care coordination, performs developmental screenings, and provides monitoring and oversight of the children’s overall health needs. This team works collaboratively with the San Mateo County Children & Family Services social workers and multidisciplinary team.

The Senior Community Worker will provide social service support to CCS clients and Pediatric Home Visiting clients and provide intensive coordination of health and health-related services to designated CCS and Children & Family Services clients through in-person field visits and electronic contacts via telephone or video calls. The Senior Community Worker may provide core services such as outreach and engagement, comprehensive assessment and care management planning, coordination of and referral to community and social support services, enhanced coordination or care, member and family support, health promotion, and comprehensive transitional care, as well as perform some administrative and/or clerical related tasks. Under the direction of the Community Program Supervisor, the Senior Community Worker will work in collaboration with both the CCS and CFS PHN teams, including the Medical Therapy Unit and Health Plan of San Mateo Whole Child Model to ensure client needs are met. Requirements of the position include knowledge and ability to easily navigate through electronic health records and provide accurate and descriptive documentation in client files.

The ideal candidate has at least two years of recent experience in supporting families in the community, conducting comprehensive assessments, care coordination, health advocacy, and referral to community services. The ideal candidate can work independently as well within a multi-disciplinary team, communicate effectively both orally and in writing and be flexible to adapting to changing environments.

The current vacancy is a Full-Time, Limited Term position. The duration of this assignment is expected to be up to three years.

This is a limited-term, at-will position. Limited Term Employees are assigned to assignments not to exceed three years. They receive medical and dental benefits, similar to regular employees, and a defined contribution retirement plan. The eligible list generated from this recruitment may be used to fill future extra-help, term, unclassified, and regular classified vacancies.

Knowledge of:

Skill/Ability to:

  • Interpret, explain, and apply administrative and departmental policies and procedures.
  • Organize, maintain, and update office database and records systems.
  • Organize own work, set priorities, and meet critical time deadlines.
  • Enter and retrieve data from a computer with sufficient speed and accuracy to perform assigned work.
  • Use tact, initiative, prudence, and independent judgment within general policy, procedural, and legal guidelines.
  • Interpret agency programs and policies to members of the community served.
  • Write clear and comprehensive reports.
  • Analyze problems, formulate plans and put them into effect with a minimum of supervision.
  • Stand and walk for extended periods of time and lift and carry equipment and supplies used in the course of the work.

Education and Experience:
Any combination of education and experience that would likely provide the required knowledge and skills is qualifying. A typical way of gaining the knowledge and skills is:
  • Two years of experience providing community services of a remedial and educational nature which includes experience counseling and conducting outreach to the client population served.
Licensure/Certification:
  • Some positions may require possession of a class C California driver license or equivalent.
Physical Demands:
Must possess mobility to work in the field as well as an office environment; to operate a motor vehicle for home visits, jail visits, to attend meetings, and visit various County sites; to inspect sites including client homes; vision and smell to assess environmental situations, identify conditions, and to read printed materials and a computer screen; and hearing and speech to communicate in person and over the telephone. The job involves fieldwork requiring frequent walking on uneven terrain and climbing and descending structures to access client homes and interact with members of the community. Finger dexterity is needed to access, enter, and retrieve data using a computer keyboard or calculator and to operate above-mentioned tools and equipment.  Positions in this classification bend, stoop, kneel, reach, and climb to perform work.  Employees must possess the ability to lift and carry materials, objects, and children weighing up to 60 pounds, with the use of proper equipment.

Environmental Elements:
Employees frequently work outdoors and are exposed to varying weather conditions, loud or excessive noise levels, unpleasant odors, dust, fumes, allergens, vermin, parasites, or insects, toxic substances; infectious agents, and cold and/or hot temperatures.  Incumbents may be exposed to blood and body fluids performing duties. Protective clothing such as gloves, masks, or gown may be necessary when performing client assessments or when supporting client job training. Employees may interact with members of the public or with staff in a highly emotional setting while interpreting and enforcing departmental policies and procedures.
