Under direction and subject to state and federal laws and regulations and professional practice standards governing the practice of pharmacy in the State of California, to select, prepare and distribute drugs, chemicals, solutions and materials for medical and scientific use; to functionally direct supportive personnel involved in the above activities; to provide clinical judgments necessary to proper utilization and administration of medications in cooperation with the medical and nursing staff; and to provide patient counseling and medication education services.

The established eligible list will be used to fill Inpatient Pharmacist vacancies at Santa Clara Valley Medical Center, O'Connor Hospital, Saint Louise Regional Hospital, and Regional Medical Center (transitioning to County ownership in April 2025) as they occur. The Inpatient Pharmacy at the four county hospitals are open 24 hours a day, 7 days a week, 365 days a year to ensure continuous and comprehensive patient care to residents of Santa Clara County. Highlighted services for each hospital include:

 

Santa Clara Valley Medical Center: Level I Trauma, Burn Center, Primary Stroke Center, Level 4 NICU, Level 2 Pediatric Trauma, Rehabilitation Center, Acute Psychiatric Services, and Comprehensive Cancer Care Services

 

O’Connor Hospital: Cancer Care Center, STEMI program, and Primary Stroke Center

 

Saint Louise Regional Hospital: Primary Stroke Center and the only acute care hospital in southern Santa Clara County.

 

Regional Medical Center: Level II Trauma, STEMI program, and Thrombectomy Capable Stroke Center (note: these are projected services to be started when the County of Santa Clara officially takes ownership in April 2025) 


  • Directs/participates in the stocking, inspection, storage, preparation, dispensing, and administration of medications in the patient care area and functionally directs assigned personnel and operations of the pharmacy area to which assigned;
  • Prepares reports on activities within the section as required;
  • Assures that strict aseptic techniques are followed in preparing sterile solutions and that records are properly completed;
  • Advises patients of drug use, giving information on the medication regarding proper use, storage, common side effects, and drug interactions.
  • Monitors the safety and efficacy of drug therapy through the maintenance of medication profiles in computerized data base;
  • Participates in management of medical emergencies, adverse drug interactions or reactions, acute and chronic disease states;
  • Participates in the education of health-care practitioners;
  • Provides drug information to prescribers and other health care practitioners; assists prescribers in the proper selection and adjustment of drug therapy;
  • Acts as pharmacist-in-charge when so designated;
  • Participates in Pharmacy/hospital committees as required;
  • Initiates and participates in drug use reviews, quality assurance programs, patient care audits, clinical drug investigations, and development of systems for improved delivery of services;
  • Develops and utilizes system for cost effective fiscal management and reimbursement;
  • Assumes the responsibilities and performs the duties of a professional pharmacist as defined the pharmacy practice provision of Business and Professional Code;
  • Performs other related duties as required.
Possession of an academic degree in Pharmacy from an accredited College of Pharmacy and a valid license to practice as a pharmacist in the State of California.

Sufficient education, training and experience to perform the above named typical tasks and to demonstrate possession of the following knowledge and abilities:

Knowledge of:
  • Prescription and non-prescription drugs and drug products and their indications, efficacy, dosage, mechanisms of action; fate and disposition (if known), major contraindications and potential side effects;
  • Influence of patient's age, sex, concomitant disease states, concurrent drug therapy, foods and diagnostic procedures on drug activity and disposition.
  • Physiology, anatomy, pathology, chemistry and mathematics as they relate to pharmacy practice;
  • Major reference sources to answer drug information/ingestion questions;
  • Pharmaceutical manufacturing methods;
  • Legislation governing pharmacy practice and hospital pharmacy services/hospital operations as they relate to pharmacy services.

    Ability to:
  • Identify, achieve, maintain and use those competencies directly relevant to his/her area of practice;
  • Establish and maintain cooperative working relationships and provide functional direction to para-professional personnel;
  • Effectively counsel patients regarding use, possible side effects and storage of medications;
  • Effectively advise and consult with medical, nursing and other personnel regarding properties of drug substances;
  • Use and evaluate information in appropriate reference sources and clinical literature.

Salary : $174,932 - $212,025
