JOB DESCRIPTION
An Aging Case Aide position for the Area Agency on Aging of Somerset County completes public contact work by assisting senior citizens in the obtainment of a wide range of services. An employee in this class title assists professional and technical social service personnel in the provision of care management by providing information and referral activities to senior citizens in order to assist them in a better understanding of community resources available to meet their individual needs. Work requires that the employee have a detailed working knowledge of the physical, cultural, and social environment of the geographical area and an understanding of the technical procedures and practices involved in evaluating individual situations. The employee will provide advice, through direct and continuous contact, to the consumer in order to assist in resolving personal problems. An individual in this class title will assist the Aging Care Manager in obtaining care plan signatures from consumers and by completing monitoring visits to assure that care needs are being met. Information and referral responsibilities will also be required. Care Management support will be provided in order to support the operations of the Care Management unit and the direct service needs of the consumer. Work is performed independently, although guidelines are provided to the employee. Supervision is provided by a professional superior and work is evaluated by the results obtained. The individual will be required to complete data entry within the Statewide reporting system (SAMS), as needed.
Specific job duties include the following:
· Searches for isolated older persons in the county through outreach efforts and/or referrals.
· Visits elderly consumers in their homes to provide information regarding services for which they are eligible.
· Provides detailed explanations to consumers relating to the procedural aspects of obtaining available services.
· Interviews consumers to discuss problems in obtaining services, refers difficult problems to an Aging Care Manager or Aging Care Management Supervisor
· Evaluate and counsel consumers in alleviating potential problems.
· Assists the Aging Care Manager in duties related to Domiciliary Care, Protective Services and other AAA programs to include the maintenance of records, filling out forms and compiling reports.
· Assists the Aging Care Manager with data entry within the Social Assessment Management System (SAMS).
· Accompanies consumers to various appointments in order to assist them in obtaining services.
· Conducts follow-up and monitoring of consumers to determine current status.
· Assures that all policies and regulations are met regarding consumer information in the Social Assessment Management System (SAMS).
· Performs Intake and Referral duties including receiving incoming calls, fulfilling information requests and documenting in SAMS as instructed.
· Provides financial information to the Fiscal Department as needed.
ELIGIBILITY – ALL CANDIDATES MUST:
· Two years of public contact work, including one year in a human services program; or
· Any equivalent combination of experience and training.
