Description:
Under direct supervision, provides support and/or client services designed to ensure effective navigation of health services to minimize fragmentation within the healthcare system and maximize quality treatment for patients of the Health Services Agency. Serves as an advocate for clients to resolve concerns and improve patient access to services across all healthcare sectors. Collaborates with healthcare providers and multidisciplinary professional staff to implement safe and efficient patient care. May perform routine patient care in a clinical setting.

- Assists patients in accessing care and navigating health care systems. Assess barriers to care and engage patients and caregivers in finding potential solutions.
- Schedules appointments for patients with various health care providers as directed.
- Coaches patients in self-management, problem solving and empower patients, family and/or caregivers to achieve maximum levels of wellness and independence.
- Bilingual Candidates: Translates, interprets and otherwise communicates with members of the Spanish speaking community.
Any combination of training and experience which would provide the required knowledge, skills and abilities is qualifying. A typical way to obtain the required qualifications would be:
- Equivalent to the completion of twelfth (12th) grade.
- Patient Navigator I: Two (2) years of experience working one-on-one assisting patients with their health needs. Must possess a clinical medical assisting certificate of completion from an accredited training institution or graduation from an accredited college or university with an Associate’s Degree in a health or social services related field.
- Patient Navigator II: In addition to the above, must possess a valid certification as a Medical Assistant from an institution approved by the California Certifying Board for Medical Assistants. Requires one (1) year of related experience in a medical setting.
Must pass a Merit Systems Services written and/or oral bilingual proficiency test or other appropriate test selected by the County) to qualify.
- Requires a valid California driver’s license.
- Must complete periodic continuing education coursework as required to maintain active certification.
- Requires CPR certification.
- Other combinations of education and experience may be considered.
