POSITION: Aging Case Aide
Full-Time; 80 hours per pay

PAY GRADE: SEIU Pay Grade 10
Starting wage: $12.31/hr.
OVERALL OBJECTIVE OF JOB
This position is responsible for locating elderly residents of Venango County, disseminating Older Adult Services service information and referring elderly residents to the appropriate service system and maintaining In Home Congregate Meals and transportation authorizations.

SUPERVISION RECEIVED
Directly supervised by the Casework Supervisor through regularly scheduled meetings, assignment of work and review of work being performed.
ESSENTIAL FUNCTIONS OF JOB
1. Provides information and referral services regarding Older Adult Services programs and activities.
2. Identifies various service organizations for the provision of agency service information and explains procedures regarding accessibility to said services through distribution of literature and brochures for referral purposes.
3. Makes initial outreach visit to elderly client’s home and provides detailed information on procedures of obtaining available services.
4. Completes the (NAT-E) needs assessment tool for in-home non-congregate meals and transportation paperwork.
5. Completes mandatory contacts by phone every 3 months and 9 months, and via face to face every 6 months and 12 months on consumers for in home non-congregate meals.
6. Completes yearly reassessments by phone for transportation.
7. Enters/updates care plans, care enrollments, and service orders for Mom’s Meals and Nutrition Inc. Meals and enters services deliveries for MOM’s Meals.
8. Enters service deliveries for transportation and meals in the SAMs database.
9. Coordinates the in-home non-congregate meals with the meal coordinator.
10. Acts as back up to clerical for follow-up calls on those not home for the meal delivery to ensure they are alright and referral to the Protective Intake Crisis Unit for those unable to locate.
11. Maintains records on all client contacts made through case recording in the SAMs database, form completion, file maintenance and monthly reporting for all Aging programs and volunteer contacts.
12. Assists eligible clients in their homes, Senior Centers, and on a walk-in basis in completing various forms necessary to secure services, i.e. PACE, Tax and Rent Rebates, Income Tax, Fuel Assistance, etc.
13. Assists Care Managers with ancillary duties as assigned by supervisor.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
1. Must be able to speak and understand the English language in an understandable manner in order to carry out essential job duties.
2. Must possess good communication and interpersonal skills.
3. Must possess ability to function independently, have flexibility and the ability to work effectively with clients, co-workers and others.
4. Must possess the ability to maintain confidentiality in regard to client information and records.
5. Must possess technical knowledge regarding basic computer usage and ability to operate office equipment.
6. Must possess knowledge of general office practices and procedures.
7. Must possess some knowledge of community resources, human service agencies and ability to deal effectively with same.
8. Must possess the ability to express compassion, empathy and patience in dealing with client, callers and visitors to the Older Adult Services office and Senior Centers.
9. Must possess flexibility to be able to assist with a variety of programs and duties as needed.
QUALIFICATIONS
High school diploma or GED and a valid PA driver’s license are required. This position also requires two years of work experience that includes general office skills and basic computer knowledge as well as public contact work related to the human services field, preferably with at least 1 year working with the elderly or volunteers.
Clearances:
· Must work in conjunction with employer to apply for Act 34 PA State Police Criminal History, Act 151 PA Child Abuse and FBI clearances prior to employment. Clearances currently possessed that are less than 4 years old will be accepted until new clearances are available. All clearances must reflect acceptable results.
· Must not appear on Preclusion Lists as defined by “Pennsylvania’s Medical Assistance Bulletin 99-11-05 Provider Screening of Employees and Contractors for Exclusion from Participating in Federal Healthcare Programs.”
· Must successfully complete pre-employment drug screening.
