POSITION: HOUSING SUPPORTS COORDINATOR
Full-Time, 80 hours/pay
DEPARTMENT: HOUSING
Human Services Complex, 1 Dale Avenue, Franklin, PA 16323
PAY GRADE: HS NON-EXEMPT HOURLY PAY GRADE ONE
Starting rate: $16.37/hr.
OVERALL OBJECTIVE
To provide support to individuals and families experiencing homelessness or who are at risk of homelessness. Works with individuals to find and secure permanent housing, assists with filling out required applications for housing and creates relationships with local landlords and apartment owners to create an inventory of possible housing solutions for homeless and at-risk individuals and families.

Receives instruction and regular supervision from Community Support Services/Housing Manager in regard to daily work duties.
ESSENTIAL FUNCTIONS OF THE JOB
1. Conducts intake or preliminary interviews to determine needs of individuals/families; performs eligibility determinations and makes appropriate referrals to other services providers.
2. Coordinates needed services for eligible individuals.
3. Creates and maintains mutually beneficial relationships with local landlords in order to have potential housing options for individuals.
4. Works all aspects with the individuals enrolled in the different housing programs including intake, assistance in locating housing, securing utilities and referrals to additional resources.
5. Assists individuals with paperwork necessary to find them stable and safe housing.
6. Assists homeless families’ transition to local shelters as required and then works with them to find a permanent housing solution. Involves working with County Service Coordinators to coordinate and support the individuals’ needs.
7. Schedules, plans, and leads team meetings and coordinates with other staff to enroll in available housing assistance programs.
8. Maintains and updates all housing program lists, data base, client records and any other necessary information required through a variety of programs.
9. Maintains accurate documentation. Prepares reports and other paperwork within timeframes mandated by federal or state regulation and/or county policy.
10. Travels frequently and makes home visits to individuals as necessary.
11. Utilizes computer to document all casework activities. Uses other office equipment such as fax machine, scanner, etc., as needed.
OTHER DUTIES OF JOB
1. Attends trainings, meetings and seminars as required.
2. Performs other job-related duties as required.

7. Must possess a valid Pennsylvania Driver’s License and daily access to reliable transportation. Travel will be throughout the county to complete assessments, monitor conditions, and provide interventions in individuals’ homes within the county.
8. Must understand and comply with all mandatory reporting requirements of CYS.
9. Must possess the principles and practices used in social service work, and the ability to apply those principles and practices within the scope of this position.
10. Must possess ability to plan and organize work and prepare adequate records and reports.
11. Must possess ability to practice organizational and stress management skills and to practice use of good judgment.
12. Must possess the ability to deliver and evaluate appropriate services to individuals and to monitor the effectiveness of such services.
13. Must possess some technical knowledge of operating personal computers and related software, or ability to learn and operate with reasonable accuracy and speed.
14. Must possess the ability to interpret regulations, policies and procedures, and apply them accordingly.
15. Must possess the ability to work with persons who have physical, mental, or emotional disabilities, who are economically disadvantaged and may be involved in the court system.
16. Must be able to interact effectively with internal agency staff, state department staff, counselors and provider agencies.
QUALIFICATIONS
Associate’s degree majoring in social welfare and 3 years of professional casework or housing/rental/building experience; or Bachelor’s degree majoring in social welfare and 1 year of professional casework or housing/rental/building experience; or Bachelor’s degree in an unrelated field with at least 12 credits in social welfare and 2 years of professional casework or housing/rental/building experience is required.
Must be able to acquire and maintain a valid Pennsylvania Driver’s License.
Clearances:
· Must work in conjunction with employer to apply for Act 34 PA State Police Criminal History, Act 151 PA Child Abuse and FBI clearances prior to employment. Clearances currently possessed that are less than 4 years old will be accepted until new clearances are available. All clearances must reflect acceptable results.
· Must not appear on Preclusion Lists as defined by “Pennsylvania’s Medical Assistance Bulletin 99-11-05 Provider Screening of Employees and Contractors for Exclusion from Participating in Federal Healthcare Programs.”
- Must successfully complete pre-employment drug screening.
