Requirements:

- Completion of an ACGME-accredited residency program in family medicine or internal medicine. ">
- Degree in medicine (either Doctor of Medicine or Doctor of Osteopathy). ">
- At least 3 years of residency training in primary care and addiction medicine. ">
- Current state medical license and state DEA license. ">
- Board certification in family medicine or internal medicine, and board certified or eligible in addiction medicine. ">
