CoveCare Center has a Full time opening for a Family Support Navigator to cover Westchester and Putnam Counties We are seeking an individual that shares our vision of partnering with individuals, families and the community to foster hope, wellness and recovery.

NYS Office of Addiction Services and Supports funds Family Support Navigators to help individuals and families understand the progression of addiction, how to navigate insurance and addiction treatment systems. FSN’s provide support throughout the continuum of care from addiction through recovery.

Family Support Navigator is someone that helps people who are struggling with the effects of addiction in a family member or friend. The objective of the navigator is to direct the client to resources in the community that are best suited for their needs. The ideal candidate should be in training or trained for a position in the Substance Use field. For example, a CASAC-T, Recovery Coach or Certified Peer Advocate. The candidate will have contacts already established in Putnam and Westchester Counties. The position requires outreach to treatment and other agencies that would have direct contact with families. This outreach would be via the phone and in person. The candidate will be computer literate with a knowledge of Word, PowerPoint and Excel.

Additionally, the Navigators will also have a presence in various community settings during the week to be a resource for the community. The expected outcome will be for Family Support Navigators to provide information about resources to help individuals and families connect with substance use treatment facilities, in-patient settings, community-based organizations, self-help and support groups, and managed care organizations so that the appropriate level and culture of care is provided.

The Family Support Navigator may receive calls directly from families and individuals during work hours, or may be assigned to follow-up from calls that come into the Family Support Navigator message line. This is a full time position requiring much time out of the office. Monthly staff meetings will take place in Cove Care Center in Carmel. Bilingual a plus.

Responsibilities:

  • Assist families struggling with addiction by connecting to treatment facilities, hospitals, community organizations and support groups.
  • Collect data, prepare reports and program outcome statements, evaluate outcomes.
  • Work with other agencies and staff member to insure integration of care.
  • Knowledge of addiction and SUD services within the Westchester, Putnam and surrounding area.
  • Supply information in a non-judgmental and linguistically appropriate manner.
  • Collaborate with community stakeholders and attend network/provider meetings on a monthly basis.
  • Inform the local community of the services available through the Family Support Navigator Program.
  • Participates and collaborates with other CoveCare Center staff during meetings and supervision;

Qualifications:

  • Minimum of 3 years’ experience in health and human services.
  • Knowledge of Substance Use Disorder, the progression of the disease and the impact it has on families.
  • Working knowledge of treatment, recovery and supportive services available in Westchester, Putnam Counties and the surrounding areas.
  • Strong communication skills
  • Ability to outreach to stakeholders in Westchester and Putnam Counties.
  • Computer Literacy
  • Bilingual Preferred
  • Bachelor’s Degree or CASAC-T or CRPA required
  • Position requires the ability to travel and use of own car.
