Job Title: In-Home Nursing Visit Nurse (Part Time/PRN)
Location: Fredericktown
Job Description: We are seeking dedicated and compassionate Registered Nurses (RN) or Licensed Practical Nurses (LPN) to provide in-home nursing visits to patients in the comfort of their homes. As an in-home nurse, you will be responsible for delivering weekly visits consisting of performing nursing assessments, weekly medication planner setup, and educating patients on managing medical conditions.
Key Responsibilities:
- Conduct in-home visits to assess the patient’s physical, emotional, and psychological health needs. Take and monitor vital signs.
- Monitor and record patients' progress and changes in condition.
- Ensure a safe environment for patients, including health and safety precautions.
- Educate patients on managing chronic illnesses, medications, and health maintenance.
- Collaborate with physicians/ Covenant Care office if any additional needs are identified.
- Maintain accurate and up-to-date patient records in compliance with HIPAA guidelines.
- Evaluation of Plan of Care for any needed updates and evaluation of care givers (aides) competency/abilities to fulfill the needs of the client per the Plan of Care.
- General Health Evaluation (semi-annual nurse visits)- Questionnaire of needs of patient.
- Additional tasks may include the below tasks based on individual’s needs and Plan of Care:
- Medication planner setup weekly
- Call in medication refills to pharmacy as needed
- Filling insulin syringes weekly for diabetics who can self-inject.
- Monitoring skin condition
- Nail Care
** All RN visit tasks are provided to persons with stable, chronic conditions, and are NOT intended as treatment for an acute health care condition as provided by a home health service (This is not a home health nursing visit) **
Qualifications:
- Nurse:
- Current and valid RN/LPN license.
- Current and valid driver’s license
- Strong assessment, critical thinking, and decision-making skills.
- Ability to work independently and manage time effectively.
- Excellent communication skills for working with patients and families.
Compensation:
- Competitive pay ranges from $35 to $38 per nurse visit.
