What are the responsibilities and job description for the Care Manager Extender, I/DD or TBI position at Covenant Case Management Services?
Covenant Case Management Services is seeking a Care Manager Extender to support Tailored Care Management (TCM) functions and enhance the care management workforce. This role will assist TCM in meeting the needs of members and help build efficient care teams. Bilingual (English/Spanish) candidates are preferred but not required. This is a hybrid/remote role.
Essential Functions and Duties:
Care Manager Extenders may assist care managers by performing the following activities. These interactions may qualify as Tailored Care Management contacts if made via phone, video, or in-person:
- Conduct general outreach, engagement, and follow-up with members.
- Coordinate services and appointments, including wellness reminders and transportation arrangements.
- Engage in health promotion activities and share knowledge as outlined in the Tailored Care Management Provider Manual.
- Communicate member circumstances to care managers and the care team.
- Provide and track referrals, assisting members in obtaining community-based resources and social support services.
- Participate in case conferences.
- Support the care manager in assessing and addressing unmet health-related resource needs.
Care Managers will direct the extender’s functions, ensuring responsibilities align with the specified scope.
Minimum Required Education:
- High school diploma or equivalent.
Minimum Required Experience/Qualifications:

At least 18 years of age
AND at least one of the following:- Lived experience with an I/DD or TBI, demonstrating knowledge of the North Carolina Medicaid delivery system.
- Lived experience with a behavioral health condition and certification as a Peer Support Specialist.
- A parent or guardian of an individual with an I/DD, TBI, or behavioral health condition with at least two years of experience navigating the Medicaid delivery system (note: a parent/guardian cannot serve as an extender for their family member).
- Two years of paid experience performing functions similar to those listed above, with at least one year working directly with the Tailored Care Management eligible population.
Physical Requirements:
- Activities include sitting, walking, typing, talking, and hearing.
- Moderate activity level with visual acuity required for data analysis and extensive reading.
- Typical office or administrative work conditions, with minimal exposure to adverse environmental conditions.
Compensation
- Starts at 18/hour
