Summary:
The Home Health and Hospice Technician is responsible for providing personal care and supplementary service as designated on the plan of treatment. The position works under the supervision of the registered nurse, LPN, or other professional for an assigned unit. Demonstrates excellent customer service performance in that his/her attitude and actions are consistent with the standards contained in the Vision, Mission and Values of Covenant HealthCare and the commitment to Extraordinary Care for Every Generation.
Responsibilities:
Demonstrates excellent customer service
H as the emotional maturity to work with dying patients and their family.
W orks at the patient’s home. The patient’s home may be a private residence, assisted living facility, a skilled nursing facility, or a hospital.
C ontributes to organization success targets for patient satisfaction and net operating margin.
P rovides assigned personal care and simple treatments to the clients to assist in the continuity of care.
F ollows the plan of care established by the Case Manager.
S upports and demonstrates assigned care measures to the family members.
A ssists the client towards independent living when possible.
R eports requests of clients and families to the supervising registered professional nurse.
R eports changes in physical status to the supervising registered professional nurse.
D evelops and maintains empathetic communications with the client and family.
C ompletes documentation requirements.
P articipates in continuing education/in-services to maintain fourteen hours per year.
O ther duties as assigned.

EDUCATION/EXPERIENCE REQUIREMENTS
H igh school diploma or G.E.D. required.
C urrent CNA certification in the State of Michigan required or obtain certification within 3 months of hire or transfer required.
B LS required.
O ne (1) year experience preferred as a CNA, HHT, HCT or related position
K NOWLEDGE/SKILLS/ABILITIES
A bility to deal effectively with the demands of the job. Patients with limited life expectancy. Ability to recognize limitations and scope of own role, and seek supervision when indicated.
A bility to read, write, and follow directions. Capable of meeting Competency Evaluation Test (OBRA-8Ab7).
A bility to perform all aspects of the role as HHT. Able to lift, turn, and transfer patients without restrictions.
K nowledge of Microsoft Excel, Word and Outlook.
V alid State of Michigan driver’s license, reliable transportation and proof of auto insurance required
W ORKING CONDITIONS/PHYSICAL DEMANDS
A bility to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
F requent standing, walking, lifting, carrying, pushing, pulling, balancing, handling, talking, and hearing.
F requent near and far vision, color and field of vision, and depth perception.
F requent lifting up to 100 lbs.
O ccasional sitting, stooping, kneeling, crouching, squatting, twisting, reaching, tasting and smelling.
