Summary:
An Advance Care Planning (ACP) Facilitator is responsible for initiating opportunities for ACP discussions at multiple encounters and throughout the lifespan of an individual within both the inpatient and outpatient settings. Demonstrates excellent customer service performance in that his/her attitude and actions are at all times consistent with the standards contained in the Vision, Mission and Values of Covenant HealthCare.
Responsibilities:
• Assess the needs of the individual seeking ACP assistance. • Design an individualized ACP approach based on the individual’s illness, readiness to participate, fears and concerns, cultural, religious, spiritual, or personal beliefs. • Make referrals to other resources as identified during the assessment. • Assist individual(s) in making informed healthcare decisions based on understanding, reflection, and discussion. • Prepare and strengthen the role of the individual’s chosen patient advocate. • Assist in the development of a plan that honors the individual’s goals, values, and beliefs for future medical care. • Develop strategies to effectively communicate the plan to those responsible for interpreting it and making decisions consistent with the individual’s goals, values, and preferences (e.g., family, physician, and other healthcare providers). • Validate proper scanning of ACP Documents. • Update Medical Decision Maker/Health Care Agent section and contact information within the EMR after validation of appropriate documents. • Assist with identifying and supporting educational opportunities with interdisciplinary team members. • Assist with increasing community awareness of ACP. • Participate in group ACP meetings. • Demonstrates commitment to customer service; serves as a role model. • Contributes to organization’s success target for patient satisfaction and net operating margin. • Work closely with Advance Care Planning Specialist in identifying areas for program improvement. • Facilitate and bill for ACP services as appropriate.
EDUCATION/EXPERIENCE REQUIREMENTS • Certified as an Advance Care Planning Facilitator required or within 1 year of hire or transfer. • RN license with two or more years’ experience in health care. • Experience in various settings, including critical care, home health, palliative care and hospice preferred. • BLS certification required. KNOWLEDGE/SKILLS/ABILITIES: • Knowledge of healthcare systems, policies, and procedures. • Ability to communicate effectively both verbally and in writing. • Ability to understand regulations concerning ethical health care practices. • Knowledge of basic computer skills in data entry and common software packages. • Demonstrates ability to establish and maintain effective interpersonal relationships and communication. • Strong organization skills with the ability to work independently and take initiative. WORKING CONDITIONS/PHYSICAL REQUIREMENTS • Ability to maintain punctual attendance consistent with the ADA, FMLA, and other federal, state, and local standards. • Constant sitting, talking, or hearing. • Frequent standing, walking, lifting, carrying, and use of hands to finger, handle and feel. • Frequent lift 0-25 lbs. • Frequent near, far, depth perception, color vision, and field of vision. • Occasional push, pull, balance, climbing, stooping, crawling, kneeling, crouching, taste or smell. • Occasional lift 26-50 lbs.
