What are the responsibilities and job description for the RN - Care Coordinator - Full Time position at Covington County Hospital?
The Care Coordinator is to promote and coordinate team-based care of patients through effective communication with patients, their caregivers/families, community resources, and their provider. Promotes high-quality care that is patient and family centered. Experience preferred.
Qualifications:
- Must be currently licensed by the state of Mississippi to practice as a registered nurse.
- Must hold current certifications in CPR and ACLS.
- Basic knowledge of community health services required.
- Ability to work independently, exercise creativity, be attentive to detail, and maintain a positive attitude required.
Duties and Responsibilities:
- Assess patient and family's unmet health and social needs.
- Provide effective communication to improve health literacy.
- Develop/maintain care plans based on mutual goals with patient, family, and provider's plan.
- Educate patients about their condition, medication, and answer questions.
- Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support specialists.
- Serve as point of contact, advocate, and informational resource for patient, family, care team, and community resources and facility.
- Other duties as assigned
