The Home Health LPN provides one on one care and skilled services to patients in their own home, contributing to the individuals overall quality of life. LPNs work under the direction/supervision of a Registered Professional Nurse, within the realm of accepted standards of practice for a Licensed Practical Nurse. The RN/LPN Team ensures the overall patient condition is cared for by completing such tasks as complex wound care and providing appropriate education and resources to the patient/caregivers, allowing them to manage their conditions and remain in their own home.

Required: Graduate of an approved License Practical Nursing program

Required: Minimum six (6) months work as a practical/vocational nurse within the last three (3) years in a hospital, nursing home or home care setting.
