POSITION: Community Health Engagement Coordinator

DEPARTMENT: Health and Human Services

REPORTS TO: Health and Human Services Director

HOURLY RANGE: $25.00 – $29.00 per hour (DOE)

JOB SUMMARY:

The Community Health Engagement Coordinator is responsible for broadening tribal engagement in health topics such as tobacco control efforts. This position coordinates, develops, and executes outreach components of the project with a special emphasis on youth; trains Tribal Youth, and provides capacity building assistance and ensures delivery of assigned day-to-day deliverables as directed by the HHS Director. Assists in writing reports, proposals, and progress reports.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Incumbent(s) may or may not be required to perform all duties listed, and may be required to perform additional tasks.

  • Schedules meetings, including arranging logistics, agenda development, recording meeting minutes and effectively communicates with staff, Tribal Council and outside agencies.
  • Seeks regular consultation of CTCP Statewide Partners to increase training and coordination.
  • Must have a deep understanding of tribal health needs and governance and in depth knowledge of health equity burdens within the tribe.
  • Assists with the formation and facilitation of the Youth Coalition.
  • Designs and develops trainings for youth coalition members.
  • In collaboration with project stakeholders, coordinates the development and implementation of assigned project activities/protocols and coordinates the delivery of services, and planning logistics.
  • Maintains expertise in project focus area(s), including staying up-to-date in research, program literature, interventions, strategies, etc. Specifically, in the areas of rural and tribal health, health equity prevention interventions, evidence- based interventions, public health strategies and capacity-building methodology, as well as intersectional issues related to poverty, stigma and tobacco addiction/education.
  • Conducts strategic planning sessions with Youth Coalition members to identify short, intermediate, and long-term goals, organizational considerations, constituents, allies, opponents, targets, and tactics.
  • Coordinates, designs, and delivers training events and other project-related technical assistance.
  • Contributes to maintenance of project activities and ensures delivery of day-to-day deliverables.
  • Contributes to the writing of the project's progress reports, summary reports, re-application proposals, technical reviews and/or new proposals that are relevant to the project.
  • Participates in team meetings, all staff meetings, and other applicable meetings, as requested. Facilitates project meetings, as needed.
  • Develops content for project newsletter, social media and marketing material.
  • Assists with the adaptation of tobacco interventions and strategies to the cultural and environmental needs of the project's stakeholders.
  • Participates in Proposition 99/Proposition 56 funded Tobacco Education/Control Coalition meetings as needed.
  • Contributes to the writing of the project’s progress reports, summary reports, re-application proposals, technical reviews and/or new proposals that are relevant to the project.
  • Travel to necessary local meetings, conferences and other trainings needed.
  • Design and implement virtual, in-person and other outreach for prevention and awareness to engage community health awareness and complete program goals, objectives and activities.
  • Participate in Tribal events including non-profit events.
  • Be a positive role model at all times.
  • Maintains professional behavior with positive representation of Coyote Valley while on and off the clock.
  • Must maintain strict confidentiality during and after employment.
  • Other duties may be assigned.

Minimum Qualifications

To perform this job successfully, an individual must be able to perform each of the essential duties and responsibilities with minimal training.   • Associates Degree in related field. Bachelor’s Degree Preferred.
  • Two (2) years relevant work experience and training that provides the skills necessary to meet the job duties.
  • Excellent verbal and written communication skills, including group facilitation and presentation.
  • Valid California Driver’s License and insurable.

Skills, Knowledge and Abilities

  • Strong organizational, interpersonal, group facilitation, and presentation skills.
  • Experience in tobacco/other advocacy, education and community strategies.
  • Able to demonstrate effective and diplomatic client service and communication skills, which includes contact with staff, tribal youth and parents/guardians, and other agencies.
  • Proficiency in Microsoft programs.
  • The ability to work independently and as part of a team.

Language Skills

  • Ability to communicate effectively in English, both written and orally.

Reasoning Ability

  • Must have the ability to deal effectively and interact well with the customers and employees.
  • Must have the ability to resolve problems/conflicts in a diplomatic and tactful manner.

The noise level in the work environment is usually low to moderate. Must be able to work in an intense environment where a high degree of concentration is necessary to perform the required job duties.
