Summary
The Cardiac Rehab Nurse provides a safe environment for the cardiac patient to regain strength following a cardiac event and to provide a knowledge base to enable the patient to achieve a healthy lifestyle in accordance with the philosophy, goals, and objectives of Cozad Community Health System.
Essential Duties and Responsibilities

· Obtains cardiac referrals, admission orders from primary and specialty (Cardiologists) providers and assess third party payer eligibility.
· Identifies cardiovascular risk factors, manifestations of disease, physical disabilities that would alter exercise tolerance, and lifestyle modification needs.
· Performs admission assessment, progress notes and discharge summaries.
· Assesses and updates plans of care.
· Implements and monitors exercise, education plans, and goals for patients.
· Participates in ongoing and systematic quality improvement efforts while providing quality healthcare services to all patients in a financially responsible manner.
· Assists Director to ensure adequate documentation and accurate charge capturing for Chargemaster.
· Follows all HIPAA guidelines and promotes and ensures patient safety.
· Fulfills mission statement by performing job duties with a high level of customer service while contributing to a positive and professional environment.
· Special projects and other duties as assigned.

Qualifications

Education and/or Experience
Graduate of an accredited school of nursing.
Licensure/Certification
Current Registered Nurse or Licensed Practical Nurse with the State of Nebraska
Basic Life Support (CPR)
Advanced Life Support (ALS)
Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively with providers, patients, visitors, and employees.
Reasoning Ability
Ability to apply common sense understanding to carry out written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.
