Summary
The Licensed Practical Nurse (LPN) is responsible for providing direct patient care, works collaboratively within the entire clinic team, and involves both clinical and non-clinical personnel. The LPN can be assigned to a specific physician, provider, or nursing team, but is expected to work on all assigned duties as identified by the Director of Clinic Operations. Assesses, plans, and implements the nursing care of patients within the facility setting with the supervision of the physician in accordance with the philosophy, goals, and objectives of Cozad Community Health System.
Essential Duties and Responsibilities

· Performs comprehensive nursing assessments efficiently and effectively.
· Accurately documents information within patient chart.
· Identifies and recognizes abnormal symptoms/changes in the patient’s condition, establishes priorities, and takes appropriate action.
· Triage calls per established provider protocols for all patients, working directly with the physicians. Documents details in EMR.
· Provides a purposeful patient and family centered partnership with the healthcare team that facilitates a seamless patient experience.
· Maximizes the patient's health outcomes and the patient’s experience by identifying patient and family needs, completing pre-visit planning, supporting the clinic visit and ensuring patient has all necessary literature, follow up appointments and lab appointments following their clinic visit.
· Schedules patient tests and referrals. Performs procedures ordered by providers including EKG or Lab.
· Performs emergency treatment as needed.
· Performs nursing department administrative duties.
· Collaborates with Medical Staff and other healthcare providers to coordinate medical and nursing management of patient care.
· Promotes and ensures patient and staff safety in all aspects of responsibilities.
· Works with other departments to provide services and resources.
· Participates in training to stay current on Rural Health Clinic regulations.
· Attends staff meetings and other educational meetings as assigned by the Director.
· Gives the best possible first impression to all patients, visitors, and employees.
· Provides quality healthcare services to all patients.
· Follows all HIPAA guidelines and promotes and ensures patient safety.
· Fulfills mission statement by performing job duties with a high level of customer service while contributing to a positive and professional environment.
· Special projects and other duties as assigned.
Supervisory Responsibilities
Supervises Medical Support Assistants. Responsibilities include training employees on all clinical functions based on competency; planning, assigning, and directing work; ensuring patient experience is fully and accurately completed; partnering with the Director of Clinic Operations to appraise performance, address complaints and resolve problems.
Education and/or Experience
Graduate of an accredited practical nursing certificate program. One year of full-time clinical practice in a rural health setting preferred. Electronic Health Records experience required.

Work Environment
While performing the duties of this job, the employee regularly works in a medical clinic setting. The noise level in the work environment is usually moderate.

Licensure/Certification
Current Licensed Practical Nurse licensure in the State of Nebraska.
Basic Life Support (CPR)
Advanced Cardiac Life Support (ACLS) preferred
