Summary
Responsible for compiling, processing and maintaining patients’ records in a hospital setting to support the daily business activities. Maintains the ability to work with Electronic Health Records ensuring all regulatory requirements of the Health System are followed in coding and charging practices.
Essential Duties and Responsibilities

· Prepares charts for scanning and scans charts into the electronic medical record system, forwarding documentation to providers as needed.
· Provides clerical support to HIM staff in relation to filing and retrieval of medical records. Assists in other department functions as assigned, such as photocopying, faxing, typing, and any other departmental task as requested by the HIM Manager. Assist in keeping provider credential files current and in compliance with Nebraska licensure laws.
· Maintains patient medical records, maintains storage, and tracks incoming records.
· Responds to requests for copies of protected health information in accordance with HIPAA privacy regulations. This includes the ability to analyze authorizations for HIPAA compliance and a working knowledge of what medium to release records on. Review medical records for completeness. This includes ensuring all necessary documentation is completed and all required fields have been signed. Electronic deficiencies are applied when necessary and monitors deficiencies and notifies providers and specialists if they have outstanding deficiencies.
· Collects and enters data into the Vital Records system in order to complete birth certificates.
· Assembles information concerning patient’s clinical background and presents appropriate and necessary information to payors to obtain authorizations for services ordered that require prior authorization and completes authorizations in a timely manner.
· Works closely with providers, coders, and billers to ensure accurate procedure and diagnosis codes are being utilized in the pre-authorization process. Ensures accurate and timely processing of all insurance claims and follows up on all prior authorization rejections.
· Responsible for queries, data consolidation, data analysis and management reporting utilizing software to generate metrics and develops actionable recommendations. Supports all areas of the hospital and clinic by collecting, analyzing data and reporting results.
· Follows all HIPAA guidelines and promotes and ensures patient safety.
· Fulfills mission statement by performing job duties with a high level of customer service while contributing to a positive and professional environment.
· Special projects and other duties as assigned.

Education and/or Experience
High School Diploma or equivalent. Ability to examine documents for accuracy. Computer experience in a healthcare setting and medical terminology knowledge required.
