Key Responsibilities
Ensure effective coordination of all aspects of care for individuals we serve, including integrated and implemented services between CPU programs.
Coordinate individualized services with Care Managers and other outside service providers, ensuring timely completion of necessary reports.
Develop and implement goal programming and activity schedules, monitoring data collection on an ongoing basis.
