Responsibilities
Some of the responsibilities of a Care Manager include,
- Complete clinical documentation in a timely and effective manner. Comply with regulatory, Agency and funding guidelines regarding documentation.
- Utilize evidenced based practices,
- provide crisis intervention and link individuals to community support.
- Assist other staff in completing assessments in a timely and comprehensive manner as required by CPC and regulatory agency policies.
- Keep up to date on the use of the Agency's electronic health record.
- Identify High Risk clients and work with team to provide safe and effective treatment outcomes.
Bilingual Spanish required
Minimul Qualifications
- Bachelor's degree in related field and at least 2 years' experience.
- A valid NJ driver's license with a satisfactory driving history is required.
