What are the responsibilities and job description for the Care Manager, Peer Support Specialist (Full-Time, Red Bank) position at CPC Integrated Health?
POSITION SUMMARY: To provide comprehensive care management and related services for individuals enrolled in the relevant agency program. The Peer Specialist will be an integral member of an interdisciplinary clinical team that will work in collaboration with the consumer in the development and execution of a care plan that will provide a holistic approach. The Peer Specialist will work with the clinical team in assessing and identifying the needs of the consumer and providing and/or arranging for health and wellness promotion services as part of the individual recovery plan in order to maximize the consumer's successful community adaptation and quality of life.
License Certification
Valid N.J. Driver's License.

- Associates degree, knowledge of recovery-based concepts, and personal history as a consumer of mental health services.
- Must have completed CPC approved Peer Specialist training at the time of hire or must have completed this training within one year of employment
