The Site Coordinator is the Day Program supervisor and oversees a team of Direct Support Providers to provide services and support to participants with Intellectual/Developmental Disabilities.

SUMMARY
Under general supervision of an Area Manager, this position oversees a residential group hojme for Adults with Intellectual /Developmental Disabilities in Sierra Vista. Duties include assessing, evaluating and supervising social services for developmentally disabled and/or seriously mentally ill participants. Incumbent is responsible for coordinating applicable service, social, vocational, educational and community services/activities with designated participants. This position is distinguished from other direct care positions by its responsibility for planning, organizing, and participating in medical, individual service plans, and social service activities directly related to ensuring the highest quality of support to participants.
WHAT YOU WILL DO:
- Responsible for activities and support services to participants at assigned site(s). Makes decisions on routine matters and consults Assistant Director on more complex issues.
- Coordinates and participates in the activities of a specialized program and assists the Scheduling Manager in maintaining effective staffing patterns to ensure quality care;
- Responsible for adhering to the site’s budgeted amounts for household goods, food and cleaning supplies;
- Communicates and interacts with participants on an individual and group basis, promoting individual choice, freedom, responsibility, and support;
- Significant participant in the hiring, separating and performance management of site staff;
- Provides direct care to participants in accordance with the individual service plan or person-centered plan, and ensures that staff assigned to the site are aware of the plan; updates staff as necessary;
- Provides on the job training and support to staff as needed, reviews care environment and appropriateness of social services, and implements changes as needed;
- Ensures a safe environment is maintained by developing and communicating procedures with participants, family members, assistant directors, visitors and staff;
- Observes participants’ behaviors and consults with primary care physicians, agencies and family/guardians regarding needed services;
- Performs ongoing evaluation of services/functions and encourages participants to share in decision-making, counseling, social, recreational, vocational, day and other activities identified in their individual service plan or person-centered plan;
- Coordinates transportation and/or escorts/provides transportation of participants to vocational, residential, and social activities, as needed;
- Implements CPES policies, standards, services and procedures;
- Takes part in developing and implementing individualized service plans or person-centered plans for participants and ensures program directives are followed in compliance with State/Federal/local standards;
- Facilitates staff and administrative meetings as well as in-service training;
- Ensures proper administration of non-prescription and prescription medication (excluding IV medications) as prescribed by licensed medical professionals;
- Serves as technical resource for staff and assists in resolving problems with the transfer of power and control from system to participant, scheduling changes, service coordination and delivery problems;
- Reviews progress notes and daily activities recorded in communication logs; ensures compliance with regulations;
- Reviews incident reports and informs appropriate staff regarding incidents involving vocational, day, social, recreational, behavioral, personal living and community services problems;
- Ensures standards of care are maintained and monitors participants’ mental and physical health to enable appropriate intervention/prevention of problems;
- Operates equipment to aid in the assistance and/or movement of incapacitated participants;
- Assists in performing various activities including counseling, crisis intervention as well as physical management procedures as necessary;
- Maintains participant records and prepares related reports and correspondence;
- Develops and maintains working relationships with all team members, including assistant directors, family members, guardians, fiduciaries, and advocates, among others.

- Knowledge, Skills and Abilities:
- Working knowledge of human behavior indicative of mental or physical disabilities, and principles and practice of self-determination;
- Strong knowledge of functions and goals of residential care, day and vocational programs;
- Ability to apply safety precautions and accident prevention techniques used in transportation and consumer care and ability to teach others the same;
- Ability to understand a household budget, reconcile receipts accurately and allocate spending appropriately;
- Knowledge of applicable State or Federal rules and regulations;
- Knowledge of CPES’s policies, practices, business goals, culture, vision, mission and values;
- Strong knowledge of community resources and social service agencies;
- Knowledge of psychiatric disorders, as well as psychotropic medications and their side effects;
- Ability to establish and maintain cooperative working relationships with professional staff, care givers, coworkers,
- and community organizations;
- Ability to work in a team environment, remaining attentive to ideas, recognizing responsibilities, and actively participating with others to accomplish tasks and achieve desired goals;
- Ability to maintain a calm, non-defensive, supportive attitude during crisis or potential crisis situations;
- Ability to recognize opportunities for learning and professional development in direct care staff and ability to coach others for success;
- Skill in using a computer for emails, timekeeping, drafting reports;
- Ability to prepare clear, concise and accurate reports and other job-related documents;
- Knowledge of HIPAA privacy and security rules;
- Ability to learn about and implement trauma-informed practices within the scope of work;
- Ability to articulate and apply program management requirements and techniques.
MINIMUM QUALIFICATIONS:
- High School diploma or GED plus two (2) years’ experience in human services/behavioral health field. OR, any combination of training and experience that demonstrates the necessary skills may substitute for the required experience;
- Preferred: Bachelor’s degree in a behavioral health field from an accredited college; or a Bachelor’s degree in any field plus one (1) year experience in behavioral health;
- Must be at least 21 years of age to meet Insured Driver requirement;
- Possess a valid class D Arizona driver’s license;
- Must be insurable under Company Fleet policy;
- Must be able to successfully meet Company training requirements;
- Have access to reliable, safe transportation with acceptable vehicle insurance to be used for agency travel as indicated by program if necessary.
- Be available by home phone, cell phone or pager for easy access by agency personnel.
- Able to work flexible schedules: weekends, nights, days, evenings, partial shifts, 8/12/16 hour shifts.
