Responsibilities :

Conduct comprehensive resident assessments using the Minimum Data Set (MDS)

Develop and implement individualized care plans based on resident needs

Coordinate care with interdisciplinary team members

Ensure accurate and timely documentation in compliance with state and federal regulations

Maintain current knowledge of MDS guidelines and best practices

Participate in quality assurance and performance improvement initiatives

Qualifications :

Registered Nurse (RN) with a current Pennsylvania license

Minimum of 2 years of experience as an MDS Coordinator in a long-term care setting

Strong knowledge of MDS 3.0 and RAI Manual

Excellent communication, interpersonal, and organizational skills

Proficient in electronic medical records systems
