Primary Responsibilities
As a member of the interdisciplinary team, the Nurse Practitioner works under the direction of the Director of Palliative Care with the primary function of providing consultation in palliative care, symptom management and supportive care to meet the needs of the seriously ill patient and family, as requested by the referring physician. The nurse practitioner will work with the interdisciplinary group who implement the plan of care.
Essential Functions
· Facilitate clarification of patient and family goals of care.
· Facilitate access to appropriate support models of care (i.e. rehabilitation, ongoing aggressive treatment).
· Assist with diagnosis, treatment and management of acute and chronic health conditions.
· Attend, participate, and lead (when applicable) internal Palliative Care’s team meetings.
· Consult with the referring physician, the acute care team and other involved providers to contribute to the patient’s plan of care.
· Prepare and maintain accurate patient records, charts and documents to support the medical practice and reimbursement for services provided.
· Prescribe medications including controlled substances to the extent delegated and licensed and after collaboration with palliative care and referring physician.
· Order diagnostics / treatments as needed after collaboration with palliative care and referring physicians.
· Interpret, evaluate and communicate diagnostic findings to the appropriate parties.
· Assist in the development of clinical practice guidelines / standards in support of quality palliative care as requested by supervisor.
· Utilize a customer service approach in all interactions with CCH teammates and patients.
· Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
· Communicates unusual findings, including those that may indicate a need for a change in the patient’s plan of care, to the medical director/physician and/or nurse case manager.
· Attends IDG conferences when required and facilitates discussion of issues to update the care plan.
· Participates in CCH orientation and in-service training programs for employees assigned to the palliative nursing service.
· Participates in CCH quality assurance reviews and evaluations of clinical services and other community programs as requested to promote professional growth and understanding of palliative care.
Knowledge, Skills, and Education Requirements
· Graduate of an accredited school of nursing and licensed to practice as a Nurse Practitioner in the state of Virginia.
· Minimum of five (5) years’ experience in home health, palliative, geriatric, oncology, or critical care.
· ACHPN certification from the Hospice Palliative Credentialing Center (HPCC) and DEA license preferred.
· Current CPR/BLS certification.
· Current driver's license in the state of Virginia.
· Interviewing and assessment skills to evaluate patient and family coping abilities.
· Demonstrated familiarity with policies of the Company rules and regulations of State and Federal bodies which aid in determining policies.
· Demonstrated knowledge of the principles of growth and development of the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in the organization's policies and procedures.
· Well-versed computer literacy with in-depth knowledge and experience with electronic medical records programs.
· Work requires the employee to travel outside the office and provide their own transpiration with mileage reimbursement from the agency.

Pay: $88,000.00 - $92,000.00 per year
