Crater District Area Agency on Aging (CDAAA) seeks a highly qualified professional to join our team as a full-time Care Coordinator. CDAAA provides senior services in the cities of Petersburg, Hopewell, Colonial Heights, Emporia, and the counties of Prince George, Sussex, Surry, Dinwiddie, and Greensville.
Key Responsibilities and Duties
The Care Coordinator is responsible for assessing, coordinating, and arranging services for CDAAA clients and ensuring appropriate community support. This role involves maintaining client charts in both PeerPlace and hard files, delivering monthly reports, staying informed of local resources, and supporting various programs as needed.

Education & Experience
- Degree in Human Services or related field.

- Capacity to quickly learn new software as required for job duties
