SUPERVISOR: RESIDENT CARE COORDINATOR/WELLNESS DIRECTOR
FLSA STATUS: NON-EXEMPT
Job Summary
The Caregiver is responsible for all duties in relation to safely providing for the resident care and well-being. It requires a mature and responsible attitude and the ability to establish confidence in the resident population regarding their care needs.
Essential Duties and Responsibilities
First and foremost, be a Sunnyside Meadows brand and culture ambassador. It is a requirement to learn and communicate Sunnyside Meadows’s Values, Mission, and Culture Keys while present in the communities.
- Responsible for fulfilling daily, weekly and monthly job checklist duties. *See attached job duties checklist.
- Understand the resident’s alert status including changes of condition and what to be aware of concerning the assistant; Report changes immediately to your supervisor or management.
- Reads, understands and appropriately documents on the 24 hour communication reports.
- Understand and agree to follow the resident’s service plan and keep updated as care needs change.
- Assists with all care needs the resident may have (bathing, transferring, toileting, ambulating, personal hygiene, dressing, etc.).
- Assist with dining room service (set up, take orders, serve, bus tables).
- Assist with activities and appointments when required and/or be responsible for getting the resident to a scheduled activity.
- Delivers and re-stocks the resident’s incontinent supplies.
- Collects and disposes of trash in designated receptacles.
- Wash, dry, fold and put away (as requested) resident personal laundry.
- Make beds neatly.
- Has knowledge and can demonstrate proper technique, after appropriate instruction with various equipment: oxygen tanks, gauges, gait belts, lifts, transfer boards, hearing aids, glasses, catheters, urine collection, transfer poles, wheel chairs, walkers, scales.
- Takes weights as instructed.
- Can complete an incident report.
- Assist with housekeeping duties as assigned: dusting, sweeping, mopping, re- stocking items, wiping down tables, chairs and hand rails, vacuuming, polishing, and cleaning bathrooms.
- Maintains general cleanliness of the resident’s apartment and reports any frayed or broken equipment to supervisor or Maintenance Director.
- Does not disrupt the resident’s belongings when in the apartment unless specifically asked to do so by the resident.
- Notifies supervisor or management if abnormal incidents or errors occur while on shift; Call for assistance if you are unsure how to manage a situation while on duty.
- Answers all call lights.
- Maintains responsibility and in good condition assigned equipment: pagers, keys, name tags, and uniform where applicable.
- Conduct yourself and the business at all times so as not to distract from, or to reflect adversely on the reputation of the Community.
- Reports to work in a safe manner and understands that potentially hazardous conditions are managed; Compliance with Safety Committee Standards is required.
- Perform other duties as assigned or needed.
Supervisory Responsibilities
This position has no supervisory responsibilities.

Education and/or Experience
- Prefer high-school diploma, or equivalent, or minimum education required per state.
- Must be able to speak English proficiently.
Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals; ability to write routine reports and correspondence; ability to speak effectively with fellow employees of the Community; Appropriate phone etiquette.
Mathematical Skills
Ability to calculate figures and amounts.
Reasoning Ability
Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form; ability to deal with problems involving several concrete variables in standardized situations.
Computer Skills
Knowledge of computerized medication administration and documentation, on-line employee training programs.

- Floor surface vary from hard tiled floor or concrete to carpet.
- Ability to tolerate a high level work pace that is typical for a senior living Community.
- The noise level in the work environment is usually moderate.
- There are times when this position will be required to assist a resident with activities
of daily living.
Other Occupational Exposures
- Blood borne pathogens require the physical abilities necessary to clean a blood spill or any other spill that may occur in this type of environment.
Other Considerations and Requirements
- Tuberculosis (TB) certificates as required by State.
- First Aid Certification as required by State.
- Criminal Background Check cleared.
Qualifications
Salary : $20
