Essential Function and Responsibilities:
Complete or assist with all intake procedures.
Develop and update appraisal needs and service plans for each client.
Prepare quarterly reports for each client.
Develop and maintain written removal or transfer record containing the information specified in Sections 87070(d) through d(3).
Ensuring provisions of or access to services and supports specified in Section 87078.1.
Provide written documentation in electronic database for contacts with clients and families, according to policy.
Protect the rights and dignity of each child.
Attend client psychotropic medication management appointments.
Transport clients and families in company or personal vehicles as needed.
Attend school related meetings, such as, Individual Education Plans and Student Study Team Meetings.
Provide crisis intervention, counseling, and case management services to families and children.
Support transition plan and prepare clients for lower levels of care, reunification, or emancipation.
Participate and present in agency trainings.
Provide on-call services after business hours.
Continually assess clients changing needs and develop plans/systems of support.
Attend Child & Family Team (CFT) meetings for each client.
Collaboration with service providers and outside agencies.
Facilitate providers meetings to support youth and families.
Supervise family visits as needed.
Perform other duties as assigned.
Physical Demands:
Must be able to bend at the waist, kneel, crouch, climb and balance.
Must be able to read, write, and visually see clients.
Must be able to hear with or without the use of hearing aid(s) sufficient to hear conversations with others, understandable voice, and speech patterns.
Must be able to physically operate office equipment sufficiently as needed.
Must be able to sit, stand and/or walk for extended periods of time.
Must have use of hands to handle and/or feel; reach with hands and arms.
Must frequently lift, push, pull or carry up to 25 lbs.

Qualifications:
Education and Experience
- Masters degree from an accredited or state approved graduate school, as defined by the Department of Education, in social work or social welfare, marriage, family and child counseling, child psychology, child development, counseling psychology, or social psychology or able to obtain an exception from Community Care Licensing
- Completion of at least three (3) semester units or 100 days of field practice or experience in a public or private social service agency at the masters degree level
- At least nine semester units of coursework related to children and families, or 18 months experience working with children and families
- At least three semester units in working with minority populations; six months of experience in working with minority populations; or six months of experience in working with minority population; or six months in-service training in working with minority populations within the first year of employment as a condition of employment
- At least three semester units in child welfare, or two years of experience in a public or private child welfare social services setting
Must be 21 years of age
Meet all state licensing and insurance requirements
Must pass a DOJ, FBI, and Child Abuse index clearance
Valid California driver's license
Good driving record: Department of Motor Vehicles printout required
Must pass LIC 503 Health Screening Report and TB clearance
