What are the responsibilities and job description for the Support Coordinator position at Creative Consulting Services?

A Support Coordinator must be able to:
- Communicate the mission and vision of Creative Consulting Services, Inc. to individuals, families, and community resources.
- Ensure individual choice is fully offered and acknowledged while maintaining a focus on safety, health, and quality of life.
- Collaborate with families, natural supports, service providers, physicians, and other community resources.
- Advocate for the needs of individuals as identified.
- Complete all required documentation accurately and on time. This includes, but is not limited to:
- Documenting all consumer and consumer-related contacts
- Reporting serious or unusual incidents
- Completing DMA-7, ISP, and SIS documentation
- Facilitate and assist in obtaining and maintaining Medicaid eligibility for individuals served.
- Facilitate ISP meetings effectively.
- Maintain a caseload of up to 40 individuals.
- Support the development of community-based natural supports for individuals.
- Participate in all required training and in-service activities.
- Monitor and respond to assigned voicemail and email regularly, ensuring responses within 24 hours.
- Maintain a professional demeanor and dress appropriately when representing Creative Consulting Services, Inc.
- Stay current on all state standards, guidelines, and policies related to service provision for individuals with developmental disabilities, as outlined in the CCS Policy and Procedure Manual.
- Demonstrate proficiency in technology and software relevant to the role, including but not limited to:
- Microsoft Excel
- Microsoft Word
- Basic internet access and navigation
- Perform additional duties as assigned by the Area Coordinator and Administrative Staff in response to contractual updates or requirements.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, or a closely related field.
- Minimum of one year of experience working with individuals with developmental or intellectual disabilities.

Pay: $44,000.00 per year
