Creative Health's role as the Core Provider within this region has allowed our Children's Service department to continually expand.
Job Summary: Creative Health Services is currently seeking a master's level candidate preferably to provide screenings and linkages to students. This position provides the candidate the opportunity to provide these services in local elementary and secondary schools within our area as well as participating on Student Assistance teams that identify students for services. Creative Health's Children Services Department focuses on developing strong working relationships with our local school districts; allowing for individualized and collaborative care across all systems.
This is a 10 month position, that starts at the beginning of the school year and ends at the end of the school year. This position is off during the summer months; however, is paid on a 12 month basis.
Job Responsibilities:
- Team Meetings: Student Assistance Liaison (SAP) team meetings occur weekly in the school setting. The liaison attends the team meeting a minimum of bi-weekly where student referrals are discussed.
- Screenings: Once a referral is made they may provide a student face to face screening with parent involvement, and follow up services with parents. The Liaisons provide the teams with updated information about community resources.
- Linkages: A linkage can occur over the phone in which the Liaison collects information from the Parent/Guardian to assess potential levels of treatment or community resources.
- MH3: Provide a total of 3 hours of psychosocial education supports to students as needed throughout the year.
- Job Requirements:
- Master's Degree highly preferred
- Bachelor's Degree in a Human Services field Required
- Child Abuse Clearance, Criminal Background Check, Act 31 Mandated Reporter, and FBI Clearances must be obtained.
- Valid Driver's License.
