DIRECT SUPPORT PROFESSIONAL (DSP) in COMMUNITY SERVICES DAY PROGRAM
Monday-Friday 8am-4pm = 40 hours
Pay rate: $18/hour
Creative Works supports adults with disabilities, at home, at work and in the community. Since 1967, Creative Works has pioneered exceptional Community Services that provide critical community-based daytime activities for adults with disabilities and autism. Our day program locations are shared spaces full of rich resources like art studios, fully-equipped kitchens where delicious meals are prepared jointly, bountiful gardens, technology centers, and exercise rooms. Our staff lead small groups of participants through a variety of activities at program and outings into the community.

JOB DESCRIPTION SUMMARY:
Responsible for the overall well-being of program participants by cultivating an environment that fosters independence within a safe learning environment and that encourages the enhancement of positive relationships.
ESSENTIAL FUNCTIONS:
Advocate for assigned individual(s).
Attend to individuals’ personal care needs. (This may include lifting and assisting individuals in and out of wheelchairs, use of Hoyer lift and maintaining their good hygiene).
Transport individuals, as needed, using personal vehicle.
Transfer from one assignment to another as requested (e.g., from one residence to another or from day program to residence and vice versa).
RESPONSIBILITIES:
Assist in developing, implementing and monitoring Individual Program Plans (IPP).
Maintain accurate records including daily logs, individual files, and accident and incident reports.
Maintain open and focused communication with team members regarding goals and progress of program participants.
Follow established schedules for both individuals we serve and those set by Team Leaders for staff.
Comply with all policies as stated in the Personnel Policies and Procedures Handbook.
Be sure any facility where individuals use CW services are kept clean and safe.
Project professional image at all times.
PHYSICAL TOLERANCES:
Ability to do standing/sedentary tasks for long periods of time.
Ability to perform range of motion tasks, including but not limited to; reaching, bending, stretching, stooping and lifting.
Ability to lift up to 50 pounds (all personnel are trained in correct lifting procedures.)
QUALIFICATIONS:
High school diploma or GED.
Experience supporting people with brain injuries and/or disabilities preferred.
Must have or be willing to obtain trainings within the Agency’s specified timeline including but not limited to: DSP, CPR/FA, CRMA, Welle, QBISP.
Must have adequate writing skills to maintain documentation standards, reports, IPPs, and any other writing expectation in the program.
Valid U.S. Driver’s License.
Satisfactory driving record per agency’s insurance company standards.
Must have ability to communicate well in the program, including the ability to read, write, speak and understand the English language so that the support of the participant receiving services can be met, and so that the relationship and work with co-workers, doctors, therapists and others in the community is successful.
