Job Summary
Mental Health Targeted Case Managers help consumers gain access to services in their community to meet their mental health needs. Case Managers help coordinate services to support areas such as, but not limited to, mental health, social, education, health, vocation, recreation, volunteer, advocacy, and transportation. Case Managers play a critical role in assisting someone with a mental health disability live independently in their community.
Duties/responsibilities include but are not limited to:
- Provide services in the four core case management components – Assessment, Planning, Referral and Linkage, and Monitoring and Coordination of services.
- Conduct Assessments
- Coordinate with a mental health professional to assure the completion of a mental health assessment of client needs and develop an individual treatment plan.
- Develop and update the client’s individualized plan (ICSP). Coordinate, monitor and evaluate services that are outlined in the plan for appropriateness, progress and continued service need.
- Discuss, research, resource and facilitate successful housing transitions for individuals
- Meet with the consumer as outlined in the treatment plan and program policies.
- Keep necessary case notes documenting contact with the client, their family and other contacts.
- Maintain client files.
- Coordinate with consumer’s team
- Advocate with Consumer
- Crisis planning and intervention.
- Manage a caseload of around 30 consumers.
Qualifications
Must meet DHS qualifications for Mental Health Practitioner in one of the following
ways:
- Has completed at least 30 semester hours or 45 quarter hours in behavioral sciences
or related fields and has at least 2,000 hours of supervised experience in the delivery
of services to adults or children with:
- Mental illness, substance use disorder, emotional disturbance, or
- Traumatic brain injury or developmental disabilities and completes training on mental
illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness, and substance abuse, and psychotropic medications and side effects.
- Has at least 4,000 hours of supervised work experience in the delivery of services to adults or
children with any of the following:
- Mental illness, substance use disorder, emotional disturbance, or
- Traumatic brain injury or developmental disabilities and completes training on mental
illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness, substance abuse, and psychotropic medications and side effects.
- Has at least 2,000 hours of supervised experience in the delivery of services to adults
or children with any of the following:
- Mental illness, substance use disorder, emotional disturbance, or
- Traumatic brain injury or developmental disabilities and completes training on mental
illness, recovery from mental illness, mental health de-escalation techniques, co-
occurring mental illness, substance abuse, and psychotropic medications and side
effects; and receives clinical supervision as required at least once a week from a mental
health professional until the requirement of 4,000 hours of supervised experience is met.
- Has a graduate student internship or a bachelor’s or master’s degree and is a graduate
student in behavioral sciences or related fields and is formally assigned by an accredited college
or university to an agency or facility for clinical training
- Has a bachelor’s or master’s degree with any of the following:
- Holds a master’s or other graduate degree in behavioral sciences or related fields
- Holds a bachelor’s degree in behavioral sciences or related fields and completes a.
practicum or internship that (1) requires direct interaction with adults or children served, and (2) is focused on behavioral sciences or related fields.
- A Mental Health Practitioner for an adult member must have training working with adults.
- Knowledge of the effects of mental illness on people with cognitive impairments.
- Knowledge of community resources.
- Must have the ability to work independently, enjoy flexibility, and manage stress.
