Recovery Support Specialist -FT CHRP
Duties And Responsibilities
- Conduct outreach to local behavioral/medical hospitals, substance recovery programs, primary care practices, school systems, social service agencies, homeless services, colleges and universities, etc. to identify persons in need of behavioral health services and facilitate their referral and enrollment for services in CREOKS.
- Establish collaborative relationships with the above institutions to develop referral networks.
- Maintain and oversee EON data collection related to the CHRP grant.
- Provide presentations to community providers and the general public about CREOKS Health Services.
- Participate in community meetings and conferences to represent and market CREOKS Health Services, Inc to other providers and the general public.
- Collect outcome data for the CHRP grant face to face or by Zoom/phone and ensure that data is reported according to the required timelines.
- Conduct outreach to persons identified as potential clients of the CHRP grant.
- Serve as the responsible staff member for persons discharging from the above-mentioned institutions until it is established that the person is engaged in CREOKS clinic-based services with a provider.
- Facilitate and teach the philosophy to staff and program participants that recovery is possible for everyone.
- Assist individuals in attempting to contact and re-engage program participants to treatment.
- Complete screening and referrals as necessary for CHRP grant.
- Assist potential clients in applying for necessary benefits to support and maintain wellness.
- Provide education on health-related issues consistent with their training and experience level.
- Attend and join community coalition(s).
- Advocate on behalf of consumers.
- Educate and assist consumers with rights, responsibilities, and appeals.
- Represent CREOKS at state and local meetings and awareness events.
- Attend weekly staff/treatment team meetings, and Care Coordination meetings as assigned by the Site Director.
- Provide screening and orient persons to behavioral health services as needed to assist with services.
- Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF and CREOKS policies
- Read and be knowledgeable of all current CREOKS policies and procedures.
- Maintain productivity averages of direct service to clientele as set by CREOKS for full time employment.
- Turn in billing and associated documentation as mandated by CREOKS policy.
- Responsible for necessary interaction with agency program participant families and/or other agencies involved with participants to ensure appropriate therapeutic care.
- Participate in mandatory training as directed by supervisor.
- Other duties as assigned.
- Must be in recovery for at least one (1) year from mental illness and/or co-occurring disorders.
- High school diploma or GED.
- Ability to conduct outreach to different providers and promote CREOKS services.
- Ability to provide presentations to the other providers and the community about CREOKS services.
- Ability to communicate needs to others.
- Ability to share story of recovery to individuals and groups.
- Knowledgeable of mental illnesses and co-occurring disorders.
- Successfully complete the ODMHSAS Peer Recovery Support Provider Training and Test to be credentialed.
- Become trained and knowledgeable of the EON data collection system.
- Successfully complete the Tobacco Free Support Group Training.
