As a Palliative Pathways Associate, you will ensure extraordinary patient and customer service by providing telephonic follow-up, engagement, and community resource support to patient referrals and their families.

Responsibilities
- Engaging with Patients and Families: Conduct telephonic outreach to patients and families, offering a compassionate ear and valuable information about available support services. Your empathetic engagement will provide comfort and clarity to those navigating difficult times.
- Providing Essential Information: Offer consistent and ongoing information about the benefits of hospice care, addressing any questions or concerns. Your expertise will help families make informed decisions about their care options.
- Coordinating Resources: Research and connect patients and families with relevant community resources and support services. By expanding our knowledgebase of support options, you will ensure that families have access to the best possible care and resources.
- Facilitating Hospice Services: Work collaboratively with Program Directors, Clinical Directors, and other colleagues to facilitate hospice evaluations and admissions. Your coordination efforts will ensure a smooth transition for patients considering hospice care.
- Building Professional Relationships: Partner with internal staff and external referral partners (MDs, Hospitals, SNFs, ALFs, etc.) to enhance communication and streamline processes. Your ability to build strong professional relationships will contribute to seamless care coordination.
Qualifications
- Associates Degree or higher Preferred; High School Diploma or equivalent required
- Experience working in post-acute healthcare, home health care, palliative care, or hospice care strongly preferred
- Proficient in Microsoft Office Suite software. Background with EMR Software preferred
- Ability to demonstrate empathy, energy, and strong customer service skills while interacting with families and patients potentially in need of hospice services.
- Prior customer service or sales experience strongly preferred.
