POSITION SUMMARY: Practitioner is a Dentist who is skilled in his/her ability to provide the full range of oral health care services including oral diagnosis, and diagnostic procedures, treatment planning, operative dentistry, fixed and removable prosthodontics, endodontics, periodontal treatment, occlusal adjustment and treatment, pediatric patient treatment and behavior management, non-surgical management of tempromandibular disorders, anxiolysis, oral surgery to include: extractions, soft tissue impactions, alveloplatsty, biopsy minor tumor removal, and treatment of minor dento-alveolar trauma. Assures clinical guidelines are followed when providing patient care. Keeps up with changing dental guidelines regarding general patient care and patient-specific clinical conditions. Discusses wellness and adherence with patients as necessary.

COMPLIANCE & QUALITY ASSURANCE STANDARDS
- Requires cooperation with the Agencys Quality Assurance and Improvement Program. Includes actively participating in all required trainings, and in creating a culture of quality improvement. Cooperation with the quality program will be considered for performance reviews.
- Requires cooperation with the Agencys Compliance Practices. Includes immediately reporting issues, actively participating in required trainings, following HIPAA regulations, and assisting in creating a culture of compliance. Also includes compliance with written standards and all applicable federal and state laws. Cooperation with compliance will be considered for performance evaluations.
- Not currently listed as excluded or sanctioned by the Department of Health and Human Services, Office of Inspector General (OIG), the General Services Administration (GSA), or excluded from participation in any federal program. Agrees to promptly notify supervisor in the event of any such debarment, exclusion, conviction, threat or indictment occurring during the term of employment.
- Failure to comply with the Agencys Compliance Practices, which include failure to report conduct or events that potentially violates any laws or the Agencys written compliance standards will be met by disciplinary action, up to and including possible termination.

KEY DUTIES AND RESPONSIBILITIES
- Examines individuals requesting care, diagnoses dental conditions, prescribes and carries out, or directs others in carrying out, appropriate dental treatment, or refers individuals for specialty consultation or treatment in conformance with approved clinical protocols and guidelines.
- Records patient-dentist transactions as they occur in the patient's dental record so that the dental record accurately and completely reflects the nature of the contact, the condition of the patient, and the care or treatment provided. In addition, the dentist shall complete patient referrals and other records and paperwork as shall be required, from time to time, by the Agency.
- Educates individuals in the nature of health-related conditions, and in the general promotion of oral health-related disease prevention.
- Assists in the provision of continuing education, on the job training and the orientation of community health center staff as requested.
- Maintains and updates skills and knowledge to appropriate professional levels with continuing education courses and educational materials.
- Responsible for personal and professional compliance with all applicable federal, state, local and center rules, regulations, protocols and procedures governing the practice of dentistry and the clinical provision of dental care as well as those relating to, but not limited to, personnel issues, workplace safety, public health and confidentiality.
- Serves on committees and attends meetings as designated by the CMO, for purposes of shaping policy and directing the clinic's operational agenda from the Dentists perspective.

Along with the Dental Office Manager, makes day-to-day decisions regarding the general operation of the Dental Clinic(s), within the confines of established policy.
- Administers or prescribes appropriate pharmacologic and non-pharmacologic treatment modalities, performs medication reconciliation;
- Utilizes evidence-based, approved practice protocols in planning and implementing care;
- Initiates appropriate referrals and consultations;
- Provides specialty specific consultation services upon request and within specialty scope of practice;
- Facilitates the patients transition between and within health care settings, such as referring, transferring, and discharging patients.
- Maintains clinical competency, as demonstrated in providing and directing patient care.
- Maintains compliance with FTCA, LSBME, OSHA and other regulatory agencies statutes and standards including CrescentCare Standards of Care and CrescentCare Clinical Staff Guidelines.
- Complies with all Agency and regulatory safety standards and infection control.
- Assists in educational programs for staff and participates in team meetings.
- Obtains proper informed consent as a prerequisite to any procedure requiring informed consent. This information must include the signature of the responsible practitioner.
- Promotes teamwork, good interpersonal relations and communication among staff at the healthcare setting.

- Operates within the scope of practice as outlined by the privileges granted and this job description.
- Participates as required in Peer Review process.
- Participate in Health Fairs, employee and volunteer health programs, immunization campaigns, and home visits as appropriate
- Assumes additional responsibilities as necessary.
POSITION REQUIREMENTS
- Received designation as DDS or DMD following successful completion of dental school curriculum from a dental school program accredited by the Commission on Dental Accreditation (CODA) of the American Dental Association
- Current BLS / ACLS certifications
- Ability to work flexible hours
- Board certification encouraged
- Holds current Louisiana Medical Licenses without restrictions
- Excellent interpersonal and communication skills required
- Unrestricted license to practice dentistry in the State of Louisiana
- Current DBA Registration
- Current LA CDS Certificate
SKILLS & COMPETENCIES
- Necessary technical skills to use electronic health record, incident reporting system, email, and other information systems.
WORK CONDITIONS
Typical Physical Demands: Prolonged sitting or standing may be required. Those physical movements and the degree of mobility normally associated with the practice of primary general dentistry will be performed on a repetitive basis. The ability to distinguish letters and symbols, as well as the ability to utilize telephones, computer terminals and copiers is required. Work under stressful conditions as well as irregular hours may be required. Frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations, radiation, and other conditions common to a clinical environment may routinely be encountered.

- Able to use a computer to chart patient information in the Electronic Health Record.
- Physical ability to participate in training sessions, presentations, and meetings.
- Some travel may be required.
