The Patient Care Manager is a key member of our leadership team. This position is responsible for ensuring our patients receive 5 star care and customer service. Registered Nurse with hospice experience required. The ideal candidate will have prior hospice leadership experience. This is an upwardly mobile position that will allow the right person to grow with the company.
We serve San Antonio and the Hill Country. Our patients are in their Homes, Personal Care Homes, Assisted Living and Long Term Care facilities. .

Salary : $87,000 - $105,000
