The Nursing Assistant/Registered delivers a wide range of resident care services, such as:

  • Caring for the residents physical, psychological, social, and spiritual needs according to the residents individualized plans of care so as to maintain or improve the functional ability and independence of residents of Crest View.
  • Maintains a clean and safe environment according to the philosophy, policies, and procedures of Crest View.
  • Maintains a commitment on the part of the organization and staff consistent with the philosophy, goals, and objectives of the organization.
  • When needed, will float to other units within Crest View.
  • Demonstrates tactfulness and positive interpersonal communication skills with personnel, residents, and visitors.
  • Maintains professional ethics and confidentiality, including upholding the Resident Bill of Rights.

RESPONSIBILITIES AND DUTIES:

Resident Care:

  • Treat residents with respect and dignity at all times.
  • Provide personal care such as showers/bathing, oral hygiene, skin care, hair care, fingernail and toenail care, toileting, and other care based on resident preferences with the objective of maintaining maximum resident functional ability.
  • Assist in serving and feeding residents, recording daily intake and output, and ensuring residents are hydrated.
  • Safely and appropriately transfers residents using a transfer belt.
  • Assists with restorative nursing procedures, including body alignment, bed positioning, and range of motion exercises.
  • Assist residents to the bathroom at least every two hours and as necessary per the care plan, following the bowel and bladder training program established for each resident.
  • Encourages and assists residents, organizing care so residents can attend activities or therapies as scheduled.
  • Observes and reports unusual symptoms, changes, accidents, and injuries to the nurse in a timely manner.
  • Reads or listens to shift report unless otherwise directed.
  • Completes temperature, pulse, respirations, blood pressure, weights, breast checks, and other vitals as needed.
  • Uses, reviews, and revises team cards under the direction of the nurse.
  • Answers call lights promptly and provides or obtains the necessary care, ensuring call lights are always within reach of residents.
  • Cares for residents requiring precautionary measures or isolation.
  • Restocks resident supplies in rooms as needed (toothbrush/paste, denture brush/paste, soap, basin, gloves, razors, etc.) during all shifts to make resident supplies and belongings are labeled.
  • Cleans, disconnects, and attaches urinary drainage equipment if applicable, draining and measuring output each shift.
  • Reports need for equipment repair to maintenance/supervisor, cleaning and caring for equipment and utensils as assigned.
  • Collects specimens as requested by nursing.
  • Serves residents food and beverages in accordance with the Director of Culinary Services procedures and expectations for serving, including serving food in both resident rooms on room trays, as well as in the dining room, in accordance with Dietitian guidance and special diets.
  • Assist residents with ambulation.
  • Prevents decubiti (pressure sores) through proper skin care and turning/repositioning residents every two hours (per care plan).
  • Records pertinent information in the EHR/EMAR via kiosks.
  • Cleans resident rooms, including stripping the bed, collecting soiled equipment or materials, and placing items in appropriate areas and emptying trash/incontinence products, soiled linens, and extra linens.
  • Cares for soiled and cleaning linen and personal resident laundry according to procedure.
  • Reports to nurse before leaving shift.
  • Completes scheduled tasks in a timely manner.
  • Is aware of and follows emergency preparedness and safety protocols.
  • Other duties as assigned.

Personal:

  • Maintains resident clinical records in compliance with facility, local, state, and federal regulations.
  • Completes timecard approval, and is present in accordance with attendance policy, and works as scheduled.
  • Attends mandatory inservices, meetings, and annual required training.
  • Acts professional in appearance and manner, wearing the official uniform, transfer belt, and nametag at all times and has appropriate personal hygiene.
  • Uses correct handwashing techniques and follows infection control processes.
  • Participates in quality improvement measures.
  • Has knowledge of and follows facility policies and procedures, Resident Bill of Rights, HIPAA, and Vulnerable Adult Act, observing confidentiality of residents, staff, and the facility.
  • Other duties as assigned.

JOB QUALIFICATIONS:

  • Must be a Registered Nursing Assistant in the State of Minnesota and maintain educational requirements to remain listed on the Nursing Assistant Registry.
  • Long-term care experience preferred
  • Ability to work with people and establish satisfactory relationships with residents, families, providers, and coworkers.
  • Ability to use time wisely, working with frequent interruptions and minimal supervision
  • Excellent verbal and written communication skills, including electronic and telephone communication

PHYSICAL QUALIFICATIONS:

  • Must be able to communicate with staff, family, and residents and to use the telephone, fax, and electronic communication methods
  • Must be able to document and maintain health records, with the ability to read typewritten documents.
  • Must be able to perform lift 25 pounds or more, and must be able to move, assist, and/or help lift residents up to 400 pounds.
  • Role requires frequent bending, squatting, stooping, sitting, walking, standing, pushing and pulling movements.
