What are the responsibilities and job description for the Social Services/Admissions Director position at Crestline Nursing Center?
Crestline Nursing Center is seeking a Social Service / Admissions Director. The position must function as a team member, team leader, and supervisor to ensure that work is accomplished, and quality care is delivered, supporting team members and taking direction from the Executive Director. This will be a dual role at two facilities!
JOB DUTIES & RESPONSIBILITIES :
- Develops and maintains a good working rapport with other direct caregivers to assure their participation and support of all programs / services to benefit the residents’ psychosocial wellbeing.
- Assist in identifying and correcting problem areas and determines the proper approach to any questions.
- Always maintains confidentiality of all resident / family information.
- Maintain proper documentation and perform general duties.
- Perform other related activities as assigned or requested.
- Assist in coordinating the referral process from the identification of the appropriate payer source to the analysis of patient information
- Collaborate with patient or representative, physicians, residents, nursing staff members, rehabilitation therapy staff members and social work staff members to ensure seamless transfer to the facility
- Communicates information effectively to physicians, supervisors, facility administrators, and any other pertinent personnel in a timely manner
- Other duties as assigned by supervisor
