Job Title: Transportation Manager
Location: Crestwood Rehabilitation & Nursing
Job Summary:
We are seeking a dedicated and organized Transportation Manager to join our skilled nursing facility team. This role ensures the smooth coordination and transportation of residents to and from medical appointments while collaborating with the clinical team to ensure all necessary documentation, medications, and equipment are prepared. The Transportation Manager plays a vital role in supporting the health and well-being of our residents by providing reliable and compassionate service.
Key Responsibilities:
- Appointment Coordination:
- Schedule and confirm medical, dental, and other healthcare-related appointments for residents.
- Communicate appointment details to residents, family members (if applicable), and facility staff.
- Transportation Services:
- Safely transport residents to and from their scheduled appointments in a timely and professional manner.
- Assist residents in and out of vehicles, ensuring their safety and comfort during transit.
- Operate the Wheelchair Lift to ensure wheelchair bound residents are able to enter and exit the vehicles safely.
- Admissions and Documentation:
- Pick up new residents admitted to the facility and safely transport them to the facility.
- Deliver all admission paperwork to the nursing staff upon arrival.
- Clinical Collaboration:
- Work closely with the clinical team to ensure residents have all necessary items for their appointments, including required paperwork, lab orders, medications, and medical equipment.
- Provide updates to the clinical team regarding appointment outcomes or additional follow-up needs.
- Record Keeping:
- Maintain accurate records of transportation schedules, resident appointments, and any associated documentation.
- Ensure all resident transportation activities comply with facility policies and state regulations.
- Vehicle Maintenance and Safety:
- Ensure the company vehicle is clean, well-maintained, and in compliance with all safety standards.
- Report any vehicle issues or maintenance needs promptly.
Qualifications:
- High school diploma or equivalent.
- Valid driver’s license with a clean driving record.
- Must be able to pass a DACS Background Check.
- Previous experience in a healthcare or transportation role preferred.
- Excellent communication and organizational skills.
- Compassionate and patient demeanor with a commitment to providing high-quality care.
- Ability to lift and assist residents as needed.
- Familiarity with HIPAA regulations and the handling of sensitive medical information.
Working Conditions:
- This position may require prolonged periods of driving and assisting residents.
- Must be able to lift up to 50 pounds.
- Flexibility to work varied shifts, including evenings and weekends, based on resident needs.
