What are the responsibilities and job description for the Social Worker position at Crestwood Health Care Center?
Crestwood Healthcare Center is currently seeking a Social Services Director for it's long term care facility.
At a minimum, the candidate must have a bachelor’s degree from an accredited school or program of social work; has two years of postgraduate experience in long-term care or related programs; and meets equivalent state requirements for social work practice, or, in jurisdictions not having such legal regulation, holds certification or credentialing from the National Association of Social Workers. In no instance shall a social worker have less than a baccalaureate degree from an accredited school or program of social work.
The qualified candidate will require a BS in Social Service and must be energetic, results orientated team leader with superior interpersonal and communication skills to take charge of the Social Services Department.
Position will involve planning, developing, organizing, implementing, evaluating and directing the overall operation of the Social Services Department. This position will also be responsible for the Medicaid process, completing MDS assessments, admission process etc.
Duties will normally involve days however occasional evening and weekend visits will be needed. We are seeking a team player who is self-motivated, enjoys working with the elderly and behavioral residents with a caring approach. Applicant needs to have 2-5 years social work and MDS assessment experience ideally in Long-term Care setting.
