The Licensed Practical Nurse (LPN) is responsible for assisting in the delivery and coordination of resident care services at the Community, as lead shift supervisor. This position also assists in monitoring the medication management program.
RESPONSIBLITIES
- Ensure compliance and understanding of all regulations regarding residents’ rights.
- Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, changing of bed linens, positioning, transfer, mobility, and incontinence care as needed.
- Assist in monitoring the medication program.
- Dispenses medication in accordance with physician orders and as stated on the Medication Administration Record for all residents.
- Since we must be staffed 24 hours per day, it may be necessary to change hours and/or days of assignment to meet the needs of our residents. We retain the right to change your work schedule as necessary, which may require mandated shifts in at times.
- Monitor and report any resident changes in condition in accordance with and rules and regulations and PSL Policies.
- Keep records updated to document completion of tasks delivered.
- Order and receive medication.
- Keep medication files and medical records current and up to date.
- Assists supervisor in transcription of medication orders from consults and telephone orders.
- Assist in giving recommendations for updating the care plans if needed.
- Assist residents in making doctor’s appointments and travel arrangements to and from appointments.
- Ensure all resident care is completed by RCA’s and assist with completion as needed.
- Perform room visits and room checks on a regular basis.
- Maintain positive relations with residents, families, and physicians.
- Meet and greet visitors in a friendly, helpful manner (Visitors include anyone who visits the community; specifically, current residents’ families and friends, prospective residents and their families, referral sources, vendors, and regulators).
- Monitor the phone, front door, call and alarm systems and respond promptly.
- Follow all local, state, and federal rules and regulations as well as PSL policies as they apply to services delivered for all residents.
- Responds to emergencies and personal emergency response system and knows when to call for backup.
- Maintain the proper certifications required by local and state licensing agencies and attend all required in-service trainings.
- Maintaining an awareness of safety hazards and unsafe practices and reporting them immediately to the appropriate supervisors when identified.
- Be a member of, on a rotating basis, on the community’s Safety Committee that include attending meetings and participating in reviewing community safety compliance and reviewing injury and accident investigations.
- Participate in annual safety training in-services.
- Performs all other duties as needed or requested by supervisor.
QUALIFICATIONS REQUIRED:
- Licensed Vocational Nurse/ Licensed Practical Nurse with a current license in good standing.
- Minimum of one year of experience working as a nurse serving a senior population.
- Knowledge of and/or ability to quickly master an understanding of related regulatory and compliance requirements required.
- Compassion for and desire to work with the elderly required.
- First Aid Certified

Pay: $27.00 - $29.00 per hour
