DAYS & HOURS:  8 hours per day; 210 days per school year

DESCRIPTION OF THE POSITION:

 

Job Title:              Youth Transition Program Specialist

Work Days:          210

Reports to:           Special Education Director

FLSA Status:         Non-Exempt

 

The Youth Transition Specialist will act as a local case manager for Youth Transition Program (YTP 2.0) eligible students.  He/She will be responsible for coordinating school based learning with work based experience, provide individual training at work site when necessary, maintain accurate case file on each YTP client, provide transportation to and from sites when necessary, act as a resource person to other school staff in the development of appropriate education/training plans, jobs, and placements.

 

ESSENTIAL FUNCTIONS:

  1. Participate in activities related to the recruitment of potential students
  2. Assist students with completing the Office of Vocational Rehabilitation Services (OVRS) application process
  3. Development of work experiences, job shadows, and career exploration activities
  4. Providing Job coaching
  5. Arranging paid employment opportunities consistent with the vocational goal of the student
  6. Job development for both work experience and career related employment
  7. Exploring occupations that compliment the students’ skills and abilities
  8. Preparing the student for employment exploration, e.g. searching for job availability, resume development, securing an application and interview with a potential employer, rehearsing interviewing skills with the student, reminding the student of proper attire when attending an interview with a potential employer, advising the student on appropriate questions to ask during an interview
  9. Ensuring that the appropriate accommodations are in place so that the student may fully participate during the meeting, assisting the student with completing the application materials, providing transportation to the orientation meeting, attending the orientation meeting as an advocate for the student and listening as the Vocational Rehabilitation counselor directs the students on possible next steps
  10. Providing coaching  regarding follow-up with potential employers
  11. Informing and facilitating referrals to other state, federal, and local vocational and related programs, e.g., Developmental Disabilities, Supplemental Security Income, Child and Family Services, Oregon Trail Card, Family Access Brokerage
  12. Gather and organize information that is useful for transition planning
  13. Maintain an accurate case file on each student
  14. Participate in transition planning meetings with students and parents
  15. Monitors progress toward meeting state performance benchmarks and data submission using the YTP state data entry system
  16. Other duties as assigned by the Director of Special Education or designee

MINIMUM QUALIFICATIONS:

 

        1.  Education:  Associate’s Degree


        2.  Knowledge: 

  • Program concepts and services for disabled individuals
  • Demonstrated ability to work effectively with a wide variety of individuals and as a team member
  • Demonstrated job development skills and knowledge of the general scope of Central Oregon employment opportunities
  • Experience in work placements and supervision of those placements
  • Personal, insured transportation, able to attain valid Oregon type 10 vehicle transportation driver’s license, acceptable driving record, and willingness to travel
  • Possess or be able to attain CRP and First Aide Certificate
  • Laws, rules, policies, and trends regarding instruction
  • Computer technology and other equipment used in the work place, as required by the position

        3.  Skills and Abilities: 

  • Possess the physical ability to regularly attend work and fulfill the essential functions of the position, with or without reasonable accommodation
  • Adhere to the federal requirements of a drug-free work place
  • Hold a valid first aid card
  • Perform the duties as required by the Teachers Standards and Practices Commission (OAR 584-20-000 to 035)
  • Read, understand and carry out oral and/or written instructions
  • Accurately transfer, record and convey information, materials and money, as required by the position
  • Communicate clearly and appropriately in speech and in writing in accordance with educational standards

WORKING CONDITIONS AND PHYSICAL EFFORT:

 

The Youth Transition Program Specialist acts as a local case manager for Youth Transition Program (YTP) eligible students.  Position requires the employee to be able to stand for up to 90 consecutive minutes and move rapidly to intervene in situations when a student's safety is being jeopardized.  Must be able to lift 50 lbs. of equipment.  Duties may also occur during times of inclement weather and varied driving conditions.

Tasks require extreme accuracy and concentration; the employee works with continual interruptions.  Must be able to sit for prolonged periods, with attention to detail and with minimal supervision.
