Key Responsibilities:
- Evaluate patients' mobility, strength, and function to identify areas for improvement
- Develop and implement individualized treatment plans tailored to each patient's unique needs
- Maintain comprehensive and accurate patient records to track progress and inform treatment decisions
- Provide patients with education on proper techniques and self-care methods to ensure optimal recovery outcomes
Requirements and Qualifications:
- At least 1 year of recent experience in a home health setting
- Degree in a relevant field (PT)
- BLS Certification (AHA)
- Current state/compact license
