In order to extend this high level of care, you will communicate with patients, families, other healthcare professionals, and social workers to promote patient safety, quality of care and cost-effective outcomes, for patients that require long-term care or home care after hospitalization. Minimum Requirements
- At least 1 year of recent acute care experience in specialty
