About the Role
As a home health hospice nurse (RN HHH), you will provide palliative care in patients' residences, focusing on finding comfort for those nearing the end of life. This may include elderly, critically ill, or disabled patients who require individualized care plans.
- Assess patients' conditions and develop personalized care plans
- Chart observations, administer medication, dress wounds, and educate patients and their families about care
- Coordinate with other medical professionals involved in patients' care plans
