Job Responsibilities:
Deliver comprehensive patient care in patients' residences, including one-on-one care for elderly, critically ill, disabled, or recovering patients.
Assess patients' conditions and develop individualized care plans, including charting observations, administering medication, and dressing wounds.
Educate patients and their families about care and coordinate with other medical professionals involved in patients' care plans.
Requirements:
At least 1 year of recent acute care experience in Home Health or a related specialty.
Active RN license.
