Requirements to apply:
- Master’s degree in nursing (NP) or Physician Assistant Studies.
- Active and unrestricted California NP or PA license (or ability to obtain).
- Current BLS/ACLS certification and DEA license.
- A passion for serving rural communities—experience in a community clinic setting is a plus!

Job Overview
As an Advanced Practice Provider, you will deliver comprehensive primary care services to patients of all ages in a supportive, team-based environment. You will utilize a holistic, patient-centered approach to address social determinants of health, promote overall wellness, and provide preventive care. Your responsibilities will include diagnosing, treating, and managing acute and chronic conditions while fostering strong provider-patient relationships. You will collaborate closely with an integrated healthcare team, including behavioral health specialists, case managers, and other medical professionals, to ensure the highest quality of care. Additionally, you will leverage technology, including Electronic Health Records (EHR), to ensure accurate and timely documentation. Patient empowerment will be a key aspect of your role, as you provide education, preventive screenings, and wellness programs to support long-term health outcomes.
