What are the responsibilities and job description for the MDS Resource position at Cross Healthcare Management?
POSITION SUMMARY
Responsible for consulting and training MDS nurses in facilities. Ensures overall nursing care is carried out as directed and required by completing timely and accurate MDS assessments per federal and state requirements. This is a resource level position that supports multiple facilities. It is a hybrid position, some remote work and some travel to facilities. The ideal candidate has 3 years of MDS experience and a proven track record of accurate documentation. The ideal candidate is a good teacher and is willing to take time to train other MDS and is looking to grow in their MDS career.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Trains MDS nurses on duties, assist with auditing, ongoing training, and providing support for facilities.
- Ensure MDS and regulartory compliance
- Identify opportunities of improvement in facilities that the MDS resource supports. Coordinate with therapy and clinical team members for improved IDT processes.
Assists MDS in their duties which include:
- Assess the health records of resident and ensure that written notations are made promptly of significant changes affecting level of activity, eating habits and physical, mental, and emotional status
- Observes and assesses the total resident focusing on all body systems, skin integrity, weight and hydration status and reports changes in resident condition promptly to supervisor
- Completes and validates all MDS assessments and writes CAA’s summaries
- Participates in planning and adjustment of Medicare assessment scheduling
- Completes Care plan and attends Care plan meetings, as scheduled
- Completes LTCMI for Medicare/Medicaid pending residents
- Completes PASARR as required for residents
- Completes/updates the Electronic documentation as required
- Maintains and updates Nursing license in compliance with state regulations
- Performs other related duties as required or requested
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
- College degree or the equivalent in work related experience
- 3 years of experience as MDS Coordinator required
- Experience with Medicaid and Medicare/HMO MDS
- Must be graduate of an accredited school of Licensed Vocational Nursing/ Registered Nursing
- Must have active state LVN/RN license
- Must be listed in good standing as confirmed on Nurse Registry
