Community Health Worker

The Community Health Worker (CHW) for Behavioral Health serves as a bridge between healthcare providers, community resources, and individuals in need of behavioral health support within a Federally Qualified Health Center (FQHC). The CHW will engage with individuals experiencing mental health and substance use challenges, providing education, advocacy, and support to improve overall well-being.

• Establish and maintain relationships with individuals in need of behavioral health services.
• Provide culturally appropriate health education, guidance, and support to individuals and families.
• Assist individuals in accessing mental health and substance use treatment, social services, and community resources.
• Conduct outreach efforts in underserved communities to promote awareness of behavioral health services.
• Support individuals in navigating healthcare and social service systems, including scheduling appointments and understanding treatment plans.
• Collaborate with healthcare providers, social workers, and community organizations to develop individualized care plans.
• Facilitate peer support groups and wellness activities to encourage social connection and resilience.
• Advocate for individuals' needs within healthcare and social service systems.
• Document and track interactions, services provided, and outcomes in compliance with program requirements.
• Participate in training and professional development activities to enhance knowledge of behavioral health topics.

Job Type: Full-time

Pay: $15.34 - $17.00 per hour
